Job Description:

Deliver Field-Based Care:</b><b> </b>Conduct comprehensive primary care visits and physical exams in the comfort of a patient&rsquo;s home.</li><li><b>
Manage Chronic Disease:</b><b> </b>Create longitudinal care plans that move the needle for high-risk patients.</li><li><b>
Collaborate Across the Continuum:</b><b> </b>Partner with an interdisciplinary team to ensure seamless, safe care transitions.</li><li><b>
Prevent ER Visits:</b><b> </b>Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.</li></ul>
Job Requirements:
<p><b>
What You Bring:</b></p><ul><li><b>
Advanced Degree:</b><b> </b>Master&rsquo;s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.</li><li><b>
Pennsylvania Credentials:</b><b> </b>Active, unencumbered NP license and Board Certification (ANCC or AANP).</li><li><b>
Reliable Mobility:</b><b> </b>Valid driver&rsquo;s license and vehicle for local field-based travel.</li></ul><div></div><div><i><br>
