Home Health Aide

We are looking for a Full or Part time Home Health Aide

Provides personal care and related non-professional services necessary to meet the needs and comfort of patients. Works under the direct supervision of the Case Manager. Reports to: Designated RN Nurse Manager

Qualifications: High school diploma or GED preferred. Successful completion of a formal training program such as STNA and/or a written skills test and competency evaluation. A training program to meet hours is available when required criteria is met. Be at least eighteen (18) years of age. Prefer one (1) year experience in a hospital, nursing home, or home health agency. Demonstrates communication skills, including the ability to read, write, and verbally report clinical information to patients, representatives, and caregivers, as well as to other agency staff. Good interpersonal skills.

Essential Functions/Areas of Accountability:

1. Renders services in strict accordance with the written Plan of Care. Reports to the Case Manager when unable to do so.

2. Performs simple range of motion exercises and/or assistance in ambulation as an extension of therapy services under the direction and supervision of the therapist.

3. Applies safety principles and proper body mechanics in the performance of specific techniques of personal and supportive care.

4. Performs personal care activities, including but not limited to bathing, shampooing, skin care/nail care, oral hygiene, shaving, and dressing, assisting with toileting/elimination., transferring and ambulation. Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home such as meal preparation/feeding, laundry, and light housekeeping and cleaning up the work area.

5. Protects patient’s right to privacy and maintains their dignity while providing personal care.

6. Prepares and provides adequate nutrition and fluid intake; provides medically prescribed diets under supervision.

7. Understands basic elements of body functioning. Observes and reports changes in patient status promptly to the Case Manager.

8. Observation, reporting, and documentation of patient status and the care or services furnished.

9. Reading and recording temperature, pulse and respiration.

10. Coordinates with other professional staff when there is a need to revise the aide care plan.

11. Participates in interdisciplinary case conferences.

12. Basic infection prevention and control procedures.

13. Recognizing and reports changes in skin condition.

14. Documents observations and care provided. Submits required paperwork per agency guidelines.

15. Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the Case Manager.

16. Encourages the patient to become as independent as possible according to the Care Plan.

17. Recognizes emergency situations and implements appropriate emergency procedures when indicated.

18. Maintenance of a clean, safe, and healthy environment. Performs housecleaning and other household services such as cleaning bathrooms/kitchens, vacuuming, dusting, cleaning floors, changing bed linens, doing laundry.

19. The physical, emotional, and developmental needs of and ways to work with the populations served including the need for respect for the patient, his or her privacy, and his or her property.

20. Performs support services per patient request and agency guidelines such as handling money, picking up prescriptions, grocery shopping, running errands and accompanying client to appointments.

21. Uses equipment/supplies responsibly. Takes care to prevent breakage in the home.

22. May perform any other task permitted under state law that is not covered in the checklist if successful training completed.

23. Must have 12 hours of in-service education annually.

  • Positive working relationships with supervisors and co-workers from all areas of the agency to maintain coordination of interrelated activities.
  • Documents legibly, timely, and according to agency documentation guidelines and standards.

Quality Improvement

  • Participates in the agency’s Quality Assurance Program and identifies practices to improve outcomes and control costs. Reports questionable activities.

Other

  • Meets attendance and punctuality expectations.
  • Consistently adheres to agency dress code and grooming guidelines.
  • Maintains agency property, supplies, and equipment in a manner that demonstrates ownership and accountability.
  • Maintains current driver’s license, reliable transportation and required automobile insurance.
  • Performs additional duties and responsibilities as deemed necessary.

Pay: $13.71 - $16.94 per hour
