About the Role
We are looking for a motivated, organized, and dependable person to join our home health team. No prior healthcare experience is required, we will train you from day one! This is a busy, multi-function role that touches HR, patient records, clinical paperwork, Veterans Affairs coordination, and vendor management. If you are a fast learner with great common sense and a strong work ethic, you will thrive here.
Key ResponsibilitiesHuman Resources & Staff Credentialing
- Assist with hiring and onboarding new staff members
- Verify and maintain employee credentials including licenses, certifications, TB tests, and background checks
- Keep personnel files organized and up to date
- Ensure staff compliance documentation is current and complete
EMR Data Entry & Medication Profile Management
- Input and update patient medication profiles in our Electronic Medical Records (EMR) system – Data Soft Logic System
- Ensure patient records are accurate, complete, and current
- Work with clinical staff to resolve any missing or incorrect information
Clinical Episode Coordination & Paperwork Follow-Up
- Track and follow up on paperwork for all Start of Care (SOC), Recertifications, Resumptions of Care (ROC), Discharges, and Follow-Up visits
- Monitor nurse route sheets and flag any missing or incomplete documentation
- Follow up with clinical staff to ensure all visit notes and physician orders are submitted on time
Veterans Affairs (VA) Authorization Management
- Track all VA patient authorizations and monitor 120-day authorization periods
- Proactively flag upcoming authorization expiration dates and initiate renewals
- Prepare and submit VA case coordinator reports
- Maintain an organized tracking system for all active VA patients
Vendor Relations & Contract Management
- Research and help contract with new vendors and service providers
- Update and maintain existing vendor contracts, tracking renewal dates
- Prepare onboarding contracts for new vendors and collect all required documentation
- Serve as the main point of contact for vendor communications

Minimum Requirements
- High school diploma or GED
- Basic computer skills (email, Microsoft Word/Excel, internet)
- Ability to pass a background check
- Strong communication skills in English
