Overview
Vital Hospice Care in Glendale, AZ is seeking a compassionate and experienced Hospice Nurse Practitioner to join our clinical team. This role is essential in providing exceptional end‑of‑life care, managing complex symptoms, and supporting patients and families with dignity and comfort. Your advanced clinical expertise will help ensure high‑quality, patient-centered hospice care throughout our service area.
Key Duties & ResponsibilitiesClinical Care
- Conduct comprehensive physical, psychosocial, and spiritual assessments to create individualized hospice care plans.
- Manage complex symptoms such as pain, dyspnea, agitation, nausea, and anxiety using evidence‑based practices.
- Perform routine, PRN, and urgent visits in patient homes or facilities to evaluate needs and adjust care plans.
- Prescribe medications, DME, and therapeutic interventions within the NP scope of practice.
- Analyze diagnostic and physiological data to respond appropriately to changes in patient condition.

Interdisciplinary Collaboration
- Work closely with the Medical Director, RN Case Managers, Social Workers, Chaplains, CNAs, and leadership staff.
- Actively participate in interdisciplinary team (IDT) meetings and contribute to care planning.
- Maintain communication with physicians, hospitals, skilled nursing facilities, and long‑term care partners to ensure continuity of care.
Patient & Family Support
- Educate families on disease progression, symptom management, medication use, and hospice philosophy.
- Provide emotional support, anticipatory guidance, and compassionate counseling during end‑of‑life decision‑making.
- Empower caregivers with the knowledge and resources needed for safe home‑based care.
Compliance & Documentation
- Complete timely and accurate charting in Kinnser/Consolo EMR according to regulatory and organizational standards.
- Ensure all documentation complies with CMS, Medicare, and HIPAA requirements.
- Participate in quality assurance activities, chart audits, and continuous improvement processes.
Additional Hospice Responsibilities
- Provide after‑hours clinical support as needed to maintain uninterrupted patient comfort.
- Assist in developing, reviewing, and improving hospice clinical protocols and procedures.
- Participate in outreach, education, or facility in‑services when appropriate.
- Maintain a professional, compassionate, and patient‑centered approach in every interaction.
Qualifications
- Active and unrestricted Arizona Nurse Practitioner license; certification in hospice care preferred.
- Experience in hospice, geriatrics, or chronic disease management strongly preferred.
- Proficiency with Kinnser/Consolo EMR or similar hospice EMR platforms.
- Strong knowledge of symptom management, pathophysiology, and hospice standards of practice.
- Excellent communication skills with the ability to navigate sensitive situations professionally and empathetically.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Commitment to continuous learning and staying updated on best practices in hospice medicine.
Compensation
Paid per visit
$120 – $180 per visit, based on experience and type of visit
Reporting Structure
This position reports directly to the Administrator/ DON.
