NP 104 Nurse Practitioner

Job Summary

We are seeking an NP 104 Nurse Practitioner to join our healthcare team, dedicated to delivering comp care to patients in the RCFE environments of assisted living and memory care. In this role, you will fulfill provider services integrated with 1:1 caregivers & professional services like RN, PT, RT and Social work. California credentials for fully independent practice and DEA registration are the minimum requirements.

Duties

• telehealth initial assessments
• Prescribe medications and treatment regimens in accordance with state laws .
• Manage acute and chronic illnesses.
• Provide telehealth consultations to expand access to care and support remote patient management.
• Collaborate with physicians, specialists, nurses, and other healthcare professionals to coordinate comprehensive patient care plans.

Qualifications

• Valid Nurse Practitioner NP 104 license for independent practice in California with current California DEA registration.

Pay: $179.00 - $180.00 per hour
