Genetics Medical Group (GMG) delivers long-term, relationship-based precision healthcare. Our clinicians build ongoing relationships with patients and use precision genetic and genomic insights to meaningfully improve care — spanning chronic disease management, metabolic conditions, mental health, and personalized medicine. We're growing our remote care team and looking for a Physician or Nurse Practitioner who wants to practice at the intersection of primary care and precision medicine.

Job Overview
As a GMG provider, you'll deliver comprehensive, longitudinal telehealth care to a patient panel — including Annual Wellness Visits (AWVs), chronic care management, and integration of precision/genetic testing insights into treatment plans. This is a fully remote, full-time position with a collaborative, mission-driven clinical team.

Responsibilities

• Conduct telehealth visits including Annual Wellness Visits (AWVs) and comprehensive E/M evaluations
• Manage patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, with a focus on hypertension, diabetes, and CKD
• Order and interpret precision/genetic testing (e.g., neurological gene panels, pharmacogenomic testing) and incorporate results into personalized care plans
• Evaluate patients presenting with neurological indications such as peripheral neuropathy, ataxia, drug-resistant epilepsy, progressive dementia, and related conditions
• Diagnose and treat common acute conditions via telehealth, including urinary tract infections (UTIs)
• Partner with GMG's clinical operations and lab partner teams to ensure accurate documentation, coding, and continuity of care
• Build ongoing, trust-based relationships with a longitudinal patient panel
• Diagnose conditions through review of symptoms lab reports, and other diagnostic tools integrated into the telemedicine system.
• Develop personalized treatment plans and prescribe medications in accordance with HIPAA regulations and clinical guidelines.
• Document all patient interactions accurately within EMR/EHR systems while maintaining strict confidentiality standards.

Requirements

• Physician (MD/DO): Active, unrestricted state medical license(s); board certification preferred (Family Medicine, Internal Medicine, or related specialty)
• Nurse Practitioner (NP): Active, unrestricted state NP license(s); board certification (FNP, AGNP, or related)
• Experience with telehealth care delivery preferred
• Familiarity with chronic disease management (CCM/RPM) and/or genetic/genomic testing workflows a strong plus
• Multi-state licensure (or willingness to obtain additional state licenses) is highly valued
• Strong documentation habits and comfort with EHR/telehealth platforms
• Excellent communication skills and a patient-centered approach to care
• Multi-state licensure, or willingness to obtain additional U.S. state licenses, is highly valued.
