Requirements: 
<ul>
<li>Bachelor’s degree in related field or commensurate experiences.</li>
<li>4 Years of Case Management Experience</li>

Responsibilities: 
<ul>
<li>Ability to handle complex case management issues and problems, Ability to identify and implement creative solutions and solve complex, challenging and changing situations.</li>
<li>Assesses treatment needs and develops care plans with mutually established goals for the people we serve Monitors and assesses client progress by conducting routine evaluations. Collects and analyzes treatment/results data, maintaining accurate client records.</li>
<li>Communicates and coordinates treatment plans to the client and the client’s guardian/family. Addresses client needs, serving as an advocate when problems and issues arise.</li>
