 What You&rsquo;ll Do
 
 
 
 The case manager will determine the resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed. This is a hybrid position. Travel will be required for service observations and team meetings. In this role, you will:
 
 
 <ul>
 <li>
 Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.
 </li>
 <li>
 Assist in the development of individual service plans and reviews in collaboration with the program participant, community service provider and other stakeholders as determined in collaboration with program participant.
 </li>
 <li>
 Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
 </li>
 <li>
 Facilitate team process for the initial and annual development of the Individual Service Plan (ISP)
 </li>
 <li>
 Ongoing monitoring of ISP and documenting results and intervene as necessary
 </li>
 <li>
 Report and review Critical Incident Reports (CIR) per protocol
 </li>
 <li>
 Participate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate
 </li>
 <li>
 Focus on Personal Outcome Measures
 </li>
 <li>
 Knowledge and understanding of Charting the Life Course framework and Person Centered Thinking/Planning
 </li>
 <li>
 Evaluate the needs of those on caseload in order to remain successful in the community
 </li>
 <li>
 Link those on caseload to programs and supports that are needed and requested to meet their needs
 </li>
 <li>
 Complete required documentation of services, including input of data, reports, and plans into Therapy

Location: Abraxas Youth Center
We are hiring a CASE MANAGER at Abraxas Youth Center. In this role, you will help improve the lives of at-risk youth in our community.
Abraxas South Mountain is located in Franklin County on Pennsylvania Route 233 between Gettysburg and Chambersburg and is surrounded by the rolling hills and beautiful woodlands of south-central Pennsylvania's Michaux State Forest.

Job Type: Full-time
Shift: 4 Day Shifts 6:45am-2:45pm, 1 Evening Shift 2:45pm-10:45pm, includes every weekend.

CASE MANAGER Job Responsibilities:<ul><li>
Coordinates and implements case management activities.</li><li>
Develops and implements treatment plans including SELF Service Plans, MAYSI, Court Reports, RPP and other reports as assigned.</li><li>
Provides weekly individual counseling and related documentation.</li><li>
Participates in, coordinates and communicates pertinent client information, RPP Meetings, and Sanctuary Meetings for their caseload.</li><li>
Writes progress/court reports as necessary and discharge summaries.</li><li>
Maintains regular contact and communicates effectively with juvenile probation officers/case workers, including them in the treatment process and updating them on clients&rsquo; progress.</li><li>
Maintains regular contact and communicates effectively with parents/legal guardians.</li><li>
Maintains client files in accordance with contractual and licensing requirements.</li><li>
Participates in team meetings.</li><li>
Ensures clients are prepared for transport to court.</li><li>
Ensures accurate billing and statistical reporting for clients on caseload.</li><li>
Ensures communication of pertinent information to outside agencies to assist in providing basic needs for clients such as IEP&rsquo;s, prior drug abuse or mental health history.</li><li>
Completes client satisfaction index at time of discharge.</li><li>
Completes referrals and intake processes.</li><li>
Assists with client behaviors and unit operations as needed.</li><li>
Supports the Abraxas philosophy and mission and promotes the Seven Key Principles of care.</li><li>
Demonstrates appropriate use of Safe Crisis Management techniques and skills.</li></ul>
Hiring Requirements:<ul><li>
High School Diploma or GED.</li><li>
Associates Degree or 60 college credits (preferred)</li><li>
Non-communicable diseases physical exam.</li><li>
At least twenty-one (21) years of age.</li><li>
Ability to work overtime as required.</li><li>
Satisfactory completion of background screening and applicable pre-employment checks; including but not limited to employment and/or personal reference and driving records.</li><li>
Ability to work with computers and the necessary software typically used by the department.</li><li>
Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.</li><li>
