Graduated from an approved school of professional nursing</li><li>
One year experience as a Nurse Practitioner is preferred</li><li>
Licensed to practice as a Nurse Practitioner in the state of agency operation</li><li>
Active DEA license in the state where care is being provided.</li><li>
Possess and maintain valid CPR certification while employed in a clinical role, preferred</li><li>
Must be a licensed driver who can travel to all business locations</li><li>
Meet the regulations and requirements of the state(s) in which program provides services</li></ul><p><b>
Responsibilities:</b></p><p>
As a Palliative Care Nurse Practitioner, you will:</p><ul><li>
Develop and implement clinical plans of care for patients with chronic and terminal illnesses</li><li>
Perform patient assessment, history and physical; recommendations on aggressive symptom-management</li></ul><ul><li>
Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching</li><li>
Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis</li><li>
Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family</li><li>
May provide support at time of death</li><li>
Prescribe medication as permitted by the state and/or the State Board of Nursing</li><li>
Participate in continum of care for patients that are hospice appropriate and ongoing education programs.</li></ul><p></p><p><b><br>

Certified by the state as a nurse&rsquo;s aide, with completion of required training and a passing score of 85% or higher on the agency&rsquo;s competency exam; additional requirements vary by state (e.g. CHHA in CA/NJ, CGNA in MD).</li><li>
CPR certification required for direct patient care in Georgia and Washington.</li><li>
Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals.</li><li>

Be the Best IT Asset Management Analyst II You Can Be</b></p><p>
The IT Asset Management Analyst II provides guidance, under limited supervision, on hardware and software asset management for the global infrastructure environment. This includes, is not limited to, gathering business requirements, reviewing the current hardware and software environment, and working closely with support teams. The IT Asset Management Analyst II will provide functional support to the IT Asset Manager, the business and other support teams in hardware and software inventory compliance. The IT Asset Management Analyst II will provide support, controlling and overseeing monthly and annual hardware inventory requirements, controlling, and approved software inventory and compliance to avoid security risk and legal pitfalls related to hardware and software asset management by understanding and leveraging industry best practices for all IT asset types.
