Job Summary Greenville Weekends
 Personal Care Aides are responsible for providing, restoring, maintaining, and promoting the health status of the clients through home support, medical monitoring, client transportation services, and assistance with activities of daily living. Our team members are expected to lead by example - maintaining a positive attitude; being a thoughtful, supportive, productive and dependable co-worker; keeping our members safe; and always working to enhance the health and happiness of the people we serve and those who care for them.
 
Qualifications
 <ul>
<li>High school diploma or equivalent.</li>
<li>At least 18 years of age for positions not requiring any driving for clients</li>
<li>At least 21 yrs of age for any positions requiring driving/running errands for clients</li>
<li>Current Home Health Aid and/or C.N.A certification preferred</li>
<li>For positions that involve driving for client (i.e. errands/Dr. appointments): Valid Driver’s license and reliable transportation and proof of vehicle registration and insurance are required</li>
<li>Minimum one-year full-time equivalent experience in a health care setting preferred.</li>
<li>Prior experience working seniors and/or adults with special needs preferred.</li>
<li>Strong communication and engaging interpersonal skills.</li>
<li>Able to multi-task and manage time effectively.</li>
<li>Able to work a flexible schedule.</li>
<li>Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds; able to stand for an extended period while providing physical assistance to members.</li>
<li>Successful results of background check, and where applicable, additional state-specific regulatory requirements.</li>

Job Responsibilities
 <ul>
<li>Provides support and assistance for members with activities of daily living and personal care as needed (e.g. arrival and departure, eating, ambulation, transferring, therapies, rest room assistance, etc.).</li>
<li>Provide home support and assists in maintaining a healthy environment by performing the following duties (i.e: light cleaning, laundry, shopping and home safety).</li>
<li>Prepare and serve meals and snacks and clear tables; assist with feeding as needed.</li>
<li>Observe members for change in condition or unusual occurrences. Escalate concerns as appropriate.</li>
<li>Run errands as needed (i.e. shopping for food, personal items, meet and/or take to appointments)</li>
<li>Ensure the safety of members by monitoring activities, keeping a sanitary, clutter-free environment.</li>
<li>Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.</li>
<li>Comply with company policies and procedures, safety and regulatory laws and standards.</li>

 Our The NEMT Driver safely transports passengers to and from non-emergency medical appointments, such as doctor visits, dialysis, physical therapy, and hospital discharges. The driver provides courteous, professional assistance while ensuring passenger safety, comfort, and on-time service in accordance with company policies and regulatory requirements.
 
Qualifications

Job Summary
 Our Drivers have a crucial role in Active Day's success. You are the first and last person with our members every day. As Transportation Coordinator you are responsible for training and mentoring our drivers. Additionally, the Transportation Coordinator develops and assigns routes, ensures fleet vehicles are in safe operating condition and maintains related records and reports. Transportation Coordinators lead by example - maintaining a positive attitude; being thoughtful, supportive, productive, and dependable co-worker; keeping our members safe, and always working to enhance the health and happiness of the people we serve and those who care for them.
 
Job Responsibilities include but are not limited to: 
<ul>
<li>Conduct Driver training - ensuring all Drivers are provided with the tools to follow all standard operating procedures.</li>
<li>Coach and mentor Drivers; provide Center Director or Transportation Manager feedback relative to Drivers’ performance.</li>
<li>Develop efficient, scheduled routes and oversee safe transport of all members promptly. Drive assigned routes and ability to jump in and drive when needed.</li>
<li>Oversee fleet; ensure Drivers conduct daily vehicle inspections, follow prescribed maintenance and fueling schedule, confirm wheelchair lifts and tie-down equipment are operating correctly, and maintain the cleanliness of their vehicles. Respond to and resolve reports of mechanical or safety concerns, repairs, or service needs.</li>
<li>Maintain current and working knowledge of state, federal, and regulatory laws & guidelines related to passenger transport.</li>
<li>Effectively manage accident investigations and related reporting.</li>
<li>Ensure proper submission of Medicaid billing claims for transportation trips, if applicable.</li>
<li>Assist Center Director and home office transportation team in identifying and implementing operational efficiencies to enhance the member experience.</li>
</ul>
Qualifications: 
<ul>
<li>High school degree or equivalent required.</li>
<li>Bilingual in English and Spanish REQUIRED</li>
<li>At least 21 years of age, and prior experience coaching, mentoring and motivating others, preferably in a transportation environment.</li>
<li>Valid driver’s license and state/vehicle-specific driver credentials.</li>
<li>Working knowledge of Microsoft Office (Outlook, Word and Excel) and GPS/routing software.</li>
<li>Strong communication and engaging interpersonal skills.</li>
<li>Able to multi-task and manage competing priorities.</li>
<li>Prior experience working with seniors and/or adults with special needs preferred.</li>
<li>Able to work a flexible schedule.</li>
<li>Able to push, pull, bend, turn, pivot, transfer and lift up to 50 pounds; able to routinely and repeatedly exit and enter the vehicle to assist members with on- and off-boarding.</li>
<li>Able to pass required physical exams including Department of Transportation (DOT) exam.</li>
<li>Successful results of background check, motor vehicle report, drug testing, and any other state-specific requirements.</li>
<li>Knowledge of local geographic area preferred.</li>
<li>Where applicable, certifications such as CPR, First Aid, and Passenger Assisted Safety & Sensitivity (PASS) are required and will be facilitated as part of job training</li>
