Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care.
The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes &mdash; in partnership with a supervising PT.
We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier &mdash; protecting your time.<h3 class="jobSectionHeader">
What you will do:</h3><ul><li>
Deliver skilled PTA visits in the home under the supervising PT&rsquo;s plan of care.</li><li>
Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.</li><li>
Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.</li><li>
Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.</li><li>
Coach patients and caregivers with practical home programs that actually get done.</li><li>
Document promptly and thoroughly in Homecare Homebase (HCHB) &mdash; defensible, efficient, and consistent.</li></ul><h3 class="jobSectionHeader">

Requirements</h3>
Must-haves<ul><li>
Active Texas PTA license (in good standing)</li><li>
Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)</li><li>
Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly</li><li>
Strong communication and documentation</li><li>
Reliable follow-through on treatment plans, visit cadence, and patient goals</li><li>
Current driver&rsquo;s license and comfort with daily travel across assigned territory</li><li>
Able to carry/transport basic therapy equipment as needed</li><li>
Competency in core home health PTA scope<ul><li>
Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device education</li><li>
Strengthening, functional mobility, and home safety assessment/modification education</li><li>
Patient/caregiver coaching with practical home programs</li></ul></li></ul>
Nice-to-haves<ul><li>
Prior home health experience</li><li>
Homecare Homebase (HCHB) experience (or similar home health EMR)</li><li>
Deeper clinical experience in high-signal ortho/neuro populations</li><li>
Strong organization and time management</li></ul></div>

Job Description:
Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.<ul><li>
Evaluate patients and perform appropriate therapy interventions</li><li>
Develop a comprehensive plan of care and coordinate multi-disciplinary care</li><li>
Perform prompt clinical documentation and visit notes</li><li>
Continued communication with physician, care team members, patient and family</li><li>
Comfortable working in all aspects of the home health setting</li><li>
Implement and maintain physical therapy protocols</li><li>
Manage patient treatment and outcomes directly, or through PTA supervision</li></ul>
Requirements:<ul><li>
Current Texas Physical Therapist (PT) license</li><li>
One year of physical therapy experience</li><li>
Experience with Homecare Homebase is helpful, but not required</li><li>
Previous Home Health experience is helpful, but not required</li><li>
Current Texas driver's license</li><li>
CPR certification</li></ul>
