Assess/monitor physical, emotional, and psychological needs of patients</li><li>
Create home health care plans that align with MD orders and the patient's goals</li><li>
Direct nursing care: administering medications, treatments, and interventions</li><li>
Provide pain and symptom management</li><li>
Educate and support the patient&rsquo;s family and caregivers</li><li>
Collaborate with an interdisciplinary team</li><li>
Maintain accurate and timely documentation</li><li>
Participate in on-call rotation as required by the local branch</li></ul>
Qualifications:
<ul><li>Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state is required</li><li>
One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred</li><li>
Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy</li><li>
New nursing graduates may be considered in select markets based on program availability</li><li>

Qualifications:
<ul><li>Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program</li><li>
