Adoration Home Health</b> is currently seeking a full time<b> LPN</b> Clinical Scheduler to support our location in <b>Cookeville, TN</b>. The ideal applicant will be an LPN experienced in home health (intermittent visits) nursing. The<b> Clinical Scheduler</b> will help support our field clinicians by scheduling their visits in the local community and will also assist with other administrative duties.</div><div></div><div><br>

Responsibilities:
<ul><li>Greets visitors in a courteous, professional, and timely manner</li><li>
Assists with answering incoming calls from patients, staff, physician offices, referral sources, etc. and transfers caller to the appropriate person or department</li><li>
Takes messages when needed and relays to the appropriate person in a timely manner</li><li>
Selects appropriate clinician for patient assignment based upon patient needs, location of the patient, and skill of the clinician</li><li>
Maintains visit data in electronic medical record</li></ul>
Qualifications:
<ul><li>High school graduate or GED
</li><li>Valid Nursing license from the state practicing in</li><li>
Current CPR certification</li><li>
Minimum of one year clinical experience, preferably in home health</li><li>
Minimum of two years of administrative experience in a healthcare environment, preferred</li><li>
Home health and/or hospice experience</li><li>
Sound knowledge of the framework, organization and function of home care</li><li>
Proficient in the use of Microsoft Office (Word, Excel, Outlook, Internet Explorer) with the ability to learn industry specific software applications
</li><li>Solid organizational skills, thoroughness, and a keen attention to detail with the ability to multi-task while prioritizing effectively
</li><li>Ability to work independently and in a team environment
</li><li>Excellent, oral, written, and interpersonal communication skills</li><li>

Perform occupational therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment</li><li>
Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function</li><li>
Maintain communication among the treatment team, physician, patient, and family members as applicable</li><li>
Maintain thorough and timely documentation of services provided to ensure proper billing</li><li>
Supervise Occupational Therapy Assistants in direct patient care and patient-related activities in accordance with state practice act</li></ul>
Qualifications:
<ul><li>Graduate of an Occupational Therapy Education program approved by CAPTE</li><li>
Current Occupational Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire</li><li>
Two (2) years of experience in adult occupational therapy, home health experience preferred</li><li>
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist</li></ul>

This would include assistance with basic personal hygiene and grooming, feeding and ambulation, medical monitoring, and other basic health care related tasks</li><li>
Home Management includes tasks that include basic housekeeping, shopping and other related housekeeping tasks</li></ul>
Qualifications:
<ul><li>Minimum age requirements are 18 years</li><li>
Valid driver's license, with satisfactory driving record, as defined by agency policy</li><li>
Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care</li><li>
Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach</li></ul>
