About the Home Health Registered Nurse Position</b></p><p><b>
Reports to: Director of Nursing</b></p><p>
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.</p><p><b>
Home Health Registered Nurse Responsibilities</b></p>
 <p><i>Patient Care</i></p>
 <ul><li>Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).</li><li>
Regularly re-evaluates patient nursing needs.</li><li>
Initiates the plan of care and makes necessary revisions as patient status and needs change.</li><li>
Uses health assessment data to determine nursing diagnosis.</li><li>
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.</li><li>
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.</li><li>
Counsels the patient and family in meeting nursing and related needs.</li><li>
Provides health care instructions to the patient as appropriate per assessment and plan of care.</li><li>
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.</li><li>
Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.</li></ul>
<p><i>Communication</i></p>
<ul><li>Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.</li><li>
Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.</li><li>
Communicates with community health-related persons to coordinate the care plan.</li></ul>
<p><i>Additional Duties</i></p>
<ul><li>Participates in on-call duties as defined by the on-call policy.</li><li>
Ensures that arrangements for equipment and other necessary items and services are available.</li><li>
Instructs, supervises and evaluates home health aide care provided every two (2) weeks.</li></ul><p><b>
Home Health Registered Nurse Qualifications</b></p>
<ul><li>Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.</li><li>
Current licensure in state and CPR certification.</li><li>
Preferred: Bachelor's degree, with one (1) year of home health care experience.</li><li>
Management experience not required. Responsible for supervising home health aides.</li><li>
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.</li><li>
Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.</li><li>
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.</li></ul></div></div><br>

<h1 class="jobSectionHeader"><b>About the Home Health Physical Therapist Position</b></h1>
<p><b>Reports to:</b> Clinical Supervisor/Director of Rehabilitation</p>
<p>The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.</p>
<h1 class="jobSectionHeader"><b>Home Health Physical Therapist Responsibilities</b></h1>
<ul><li>Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:<ul><li>
Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.</li><li>
Assesses for muscle strength, mobility, gait, ROM&mdash;potential for rehab.</li><li>
Directing physical therapy treatment.</li><li>
Instructing patients and families/caregivers in the use and care of therapeutic appliances.</li><li>
Determining priority needs for physical therapy.</li><li>
Reporting to physician patient's reaction to treatment or changes in condition.</li></ul></li><li>
Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.</li><li>
May train patient in the use of prosthetic device.</li><li>
Identifies patient and family/caregiver needs for other home health services and refers as necessary.</li><li>
Prepares and submits clinical and progress summaries based on the attainment of goals.</li><li>
Participates in discharge planning for patient.</li><li>
Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.</li><li>
Provides physical therapy consultation to home families/caregivers when indicated.</li><li>
Provides inservice education programs for nursing organization personnel as needed.</li><li>
Participates in peer consultation process.</li><li>
Supervises physical therapy assistants according to organization policy and state regulations.</li><li>
When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.</li></ul>

<h1 class="jobSectionHeader"><b>Home Health Physical Therapist Qualifications</b></h1>
<ul><li>Possesses a Position Qualifications degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization.</li><li>
Licensed to practice as physical therapist within the state, if applicable.</li><li>
Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.</li><li>
Demonstrates good verbal and written communication and organization skills.</li><li>
Possesses and maintains current CPR certification.</li><li>
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.</li></ul>
