</b></p></div><div><p>Responsible for providing direct and comprehensive administrative support to the hospice IDG clinical team, which encompasses medical records, scheduling, QA activities, and IDG documentation support.<br>
</p></div><p></p><div><p><b>Hours / Schedule: </b>Full Time; (40) hours / week.<br>
</p></div><p></p><div><p><b>Minimum Qualifications:
</b></p></div><div><ul><li><p>Diploma/Degree in nursing with a Licensed Practical Nurse (LPN) or related field from an accredited college or university or the international equivalent is preferred.
</p></li></ul></div></div><div><div><ul><li><p>Licensed Practical Nurse (LPN) is preferred.
</p></li></ul></div><div><ul><li><p>One (1) year experience in clinical records management, preferably in hospice care operations.
</p></li></ul></div><div><ul><li><p>Information systems knowledge required .
</p></li></ul></div><div><ul><li><p>Accredited Records Technician Certification preferred.<br>
</p></li></ul></div><p></p><div><p><b>Key Responsibilities:
</b></p></div><div><ul><li><p>Manage all daily clinical records functions including establishing and implementing clinical records policies.
</p></li></ul></div><div><ul><li><p>Ensure clinical record systems are maintained in compliance with state, federal, and ACHC regulations.
</p></li></ul></div><div><ul><li><p>Maintain comprehensive working knowledge of state, federal, and ACHC documentation regulations and serves as a resource for appropriate organization personnel.
</p></li></ul></div><div><ul><li><p>Protect all clinical records through the establishment and implementation control procedures for all open and closed records.
</p></li></ul></div><div><ul><li><p>Protect the confidentiality of patient and organization information through effective controls and direct supervision of clinical records.
</p></li></ul></div><div><ul><li><p>Forward copies of clinical records to authorized users according to policy.
</p></li></ul></div><div><ul><li><p>Complete an administrative record audit following patient discharge and forward the record to the Clinical Manager for clinical audit.
</p></li></ul></div><div><ul><li><p>Monitor aged unsigned physician orders and submit second and third requests for signed orders to overdue physicians.
</p></li></ul></div><div><ul><li><p>Call or visit the offices of seriously overdue physicians.
</p></li></ul></div><div><ul><li><p>Supervise the use of the clinical records information system and maintain a comprehensive working knowledge of the system including upgrades and enhancements.
</p></li></ul></div><div><ul><li><p>Recruit, select, orient, and manage members of the clinical records team.
</p></li></ul></div><div><ul><li><p>Manage the members of the clinical record team to ensure accurate and prompt filing and retrieval of clinical records.
</p></li></ul></div><div><ul><li><p>Responsible for providing direct support and assistance related to scheduling activities as assigned
</p></li></ul></div><div><ul><li><p>Work directly with clinical, administrative, and leadership team members to ensure scheduling assignments are established , communicated, and coordinated to support the needs of patients and families as outlined via the Plan of Care and IDG.
</p></li></ul></div></div><div><div><ul><li><p>Perform other Clinical Support duties as assigned by management to support department goals and initiatives.
</p></li></ul></div><div><ul><li><p>Demonstrate the ability to collaborate with an IDG member.
</p></li></ul></div><div><ul><li><p>Knowledge of corporate business management, governmental regulations and general hospice operation standards.
</p></li></ul></div><div><ul><li><p>Demonstrate good communication skills and public relation skills.
</p></li></ul></div><div><ul><li><p>Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
