Aga Khan Health Service, Pakistan, is looking for a competent <b>Health Facility Coordinator</b> based in <b>District Diamer.</b></p>
<p><b>
Key Responsibilities:</b></p>
<ul>
<li>Support the health facility team, including LHVs and vaccinators, in nutrition screening and immunization activities at the health facility</li>
<li>Build and sustain strong coordination with the district EPI and WHO/UNCEF team, project staff, and health facility team to ensure the provision of integrated immunization and nutrition services</li>
<li>Support the field teams in day-to-day activities, ensuring alignment with project objectives and consistency in service delivery across all health facility catchment areas.</li>
<li>Support capacity-building initiatives and training sessions for facility staff on program guidelines, data handling, and integrated service delivery</li>
<li>Monitor and improve service quality by helping the team to adhere to program protocols, providing feedback and support as needed.</li>
<li>Coordinate with health facility staff to ensure timely and high-quality collection and reporting of immunization and nutrition-related data using the project application while ensuring data accuracy and compliance with program requirements.</li>
<li>Provide technical assistance to HF staff on using the digital application.</li>
<li>Generate reports on immunization and nutrition indicators for health facilities and stakeholders</li>
<li>Conduct regular data quality checks to identify and resolve discrepancies.</li>
<li>Ensure availability of equipment, nutrition, and medical supplies at health facilities in coordination with the district coordinator and Nutrition and MCH Officer for uninterrupted service delivery</li>
<li>Ensure proper storage, handling, distribution, and record of nutrition supplies at the health facility</li>
<li>Any other task assigned by the program team</li>
</ul>
<p><b>Qualifications &amp; Experience:</b></p>
<ul>
<li>Diploma in nursing/LHV/midwifery or at least bachelor&rsquo;s degree in social sciences or a related health field</li>
<li>Minimum 6 months to 1year of experience in data collection and reporting, health facility coordination, community health programs, or social mobilization.</li>
<li>Ability to work under pressure with pressing deadlines.</li>
<li>Should be polite, respectful and courteous to the patients, community and to those who are in authority.</li>
<li>Strong organizational, communication, and interpersonal skills.</li>
<li>Ability to speak and understand local languages</li></ul><br>

Position Summary:</b></p>
<p>
The role oversees field activities, supports supervision and performance management of frontline teams, and maintains coordination between communities, health facilities, and key stakeholders. The Project Coordinator will also support additional regional AKHSP initiatives, ensuring objectives are met, data is accurate, and implementation issues are addressed promptly.</p>
<p><b>
Key Responsibilities:</b></p>
<ul>
<li>The Project Coordinator is accountable for execution, coordination, supervision, and reporting of CASI initiatives. Key responsibilities include:</li>
<li>Coordinating and overseeing CASI implementation across Southern region in line with approved work plans, AKHSP standards, and national nutrition guidelines.</li>
<li>Supervising, mentoring, and managing performance of field teams (RN/LHV&rsquo;s), providing technical support where needed.</li>
<li>Acting as the main point of contact for Regional and Local Health Boards, district health offices, and local authorities, providing timely updates on implementation, risks, and corrective actions.</li>
<li>Supporting planning and execution of additional regional nutrition or health initiatives as assigned.</li>
<li>Ensuring functional referral pathways between communities and health facilities.</li>
<li>Overseeing monitoring, data collection, documentation, and timely reporting, ensuring accuracy and compliance with organizational standards.</li>
<li>Managing proper use and tracking of project resources, supplies, and materials.</li>
<li>Managing project finances and keeping a track of expenditures.</li>
<li>Applying safeguarding, accountability, and community participation standards consistently across all activities.</li>

<p><b>Qualification, Experience &amp; Skills Required:</b></p>

<li>Hold a bachelor&rsquo;s or master&rsquo;s degree in Public Health, Social Sciences, Nutrition, Management, or a related field.</li>
<li>Have 2 + years of experience coordinating or managing field-based nutrition, health, or community programs.</li>

Job Summary:</b> Case Management Officers act as the frontline responders of the mental health helpline, serving as the first point of contact for all callers. They are responsible for handling incoming calls, conducting initial assessments (including risk evaluation), and providing immediate emotional support. They ensure that each case is properly documented and referred to the appropriate care based on need, language, and complexity. Additionally, they maintain accurate records, follow up on high-risk cases, and contribute to regular reporting while ensuring strict confidentiality and ethical standards. Along with the helpline management, CMOs are responsible for regular loss to follow cases and ensure strong inter AKDN agency coordination for closure of the cases on completion of the services required. Aga Khan Health Service, Pakistan, is looking for a competent <b>Case Management Officer</b> based at <b>Helpline Centre</b>, Gilgit, in Gilgit Region.</p>
<p><b>
Key Responsibilities:</b></p>
<ul>
<li>Enter cases received through the mental health helpline and take consent for further deliberations and actions.</li>
<li>Review newly registered cases through different entry points (helpline, community, health service etc) and verify completeness of intake data in the Centralize Case Management System (CMS).</li>
<li>Maintain and update the call log system OR centralize case management system and ensure confidentiality.</li>
<li>Confirm consent status and screening documentation + Schedule and document re-engagement for cases where consent is initially declined.</li>
<li>Review system-generated risk stratification (high, moderate, low) and operationalize level of care in line with the stepped-care model. Provide immediate emotional support and crisis de-escalation.</li>
<li>Ensure response timelines are met for all risk levels.</li>
<li>For Crisis &amp; Emergency Response, including Helpline, respond to automated alerts for emergency and high-risk cases.</li>
<li>Conduct crisis assessment and triage as per protocol.</li>
<li>Activate crisis and emergency referrals and document actions in real time.</li>
<li>Ensure continuity of care post-stabilization.</li>
<li>Initiate and track referrals to psychologists, psychiatrists, and any other services.</li>
<li>Monitor referral acceptance and completion.</li>
<li>Coordinate with inter-agency partners to ensure continuity of care.</li>
<li>Ensure follow-ups are conducted according to risk-based schedules.</li>
<li>Re-initiate clinical or crisis pathways if risk persists or escalates.</li>
<li>Monitor CMS flags for LFTU cases and initiate re-engagement and escalate unresolved cases for supervision.</li>
<li>Participate in regular case management supervision.</li>
<li>Ensure confidentiality of the patients or the cases.</li>
<li>Contribute to dashboards and reports through accurate CMS data entry.</li>

<p><b>Qualifications &amp; Experience:</b></p>

<li>Minimum BScN/MScN or BS/MSc/MS in social sciences.</li>
<li>Minimum 2 to 3 years of related experience particularly in patient care/management, follow-ups, interaction.</li>
