Age Well Arrowhead Transportation Provider - Job DescriptionPURPOSE: To provide medical and nonmedical transportation to older adults living in Duluth, Hermantown, Proctor, and Superior.REPORTS TO: Program ManagerCASUAL EMPLOYMENT DEFINITION: There is no guarantee or commitment from Age Well Arrowhead that work will continue indefinitely;&middot; Compensation is only for time worked; &middot; Loaded mileage is reimbursed at the federal rate; &middot; Casual employees can end employment without advanced notice.Age Well Milestone Bonus Program: $250, $500, and $1000 annuallyPOSITION DEMANDS:&middot; Pick up client at their home and transport them to and from their appointment; &middot; Availability 2-3 days per week or more &middot; Excellent communication skills, including verbal and written communication; &middot; Accompaniment and companionship to appointment location when required &middot; Team player who understands how individual contributions are essential for the success of the program/agency as a whole and who values professionalism; &middot; Flexible schedule to accommodate client transportation needs; &middot; Friendly, patient reliable &middot; Reliable and safe vehicle with up-to-date auto insurance; &middot; Driver's license with personal vehicle.PRINCIPAL RESPONSIBILITIES:&middot; Provide transportation; &middot; Companionship; &middot; Follow scheduled pick-up times; &middot; Provide safe and reliable vehicle; &middot; Driving clients as requested; &middot; Effectively communicate with client; &middot; Comfortably navigate Duluth and Superior locations; &middot; Provide appropriate record keeping per position requirements; &middot; Maintain client records as required by funding source(s), HIPAA and AAAA requirements; &middot; Perform transportation duties as assigned; &middot; Log hours using Better Impact app; &middot; Provide information about each client visit using Better Impact; &middot; Participate in mandatory meetings and trainings; &middot; Ability to use transportation scheduling program; &middot; Report to work on time; &middot; Communicate any concerns regarding the client or your position immediately to the Program Manager; &middot; Have a backup plan for unanticipated events that may prevent you from reporting to work.QUALIFICATIONS:&middot; High school diploma; &middot; Ability to pass a background check; &middot; Use of personal vehicle with up-to-date auto insurance; &middot; Experience working with older adults; &middot; Ability to work independently and interdependently; &middot; Ability to communicate effectively both orally and in writing; &middot; Ability to be trained in new models of support; &middot; Ability to utilize personal computer for recordkeeping, reporting, communication &middot; Ability to articulate and support Age Well mission and vision; &middot; Valid Drivers License; &middot; Clean driver record is required &middot; Clean and reliable vehiclePERFORMANCE IS SATISFACTORY WHEN: Client satisfaction measures report that the program is responsive to their needs; records are complete, accessible, and organized.Pay: From $17.00 per hourBenefits:<ul><li>Flexible schedule</li><li>Paid training</li><li>Safety equipment provided</li></ul>Work Location: In person
