<div>Summary of Position 
The Destination Home (DH) Case Manager has the responsibility to support ASD&rsquo;s intermediate and long-term residential stability of all house assigned residents. Promote the organization&rsquo;s mission and goals, to respect the confidentiality of all who come to us for information or assistance. And to personally maintain the dignity and integrity of one who is placed in a position of public trust. The DH Case Manager continuously works to help improve the organization&rsquo;s open communication.
The focus of this position will be to ensure efficient delivery of quality case management services to clients through assessment, planning, service procurement, delivery, coordination, and monitoring, ensuring all services are offered in an inclusive, respectful and professional manner. The DH Case Manager reports directly to the DH Housing Program Manager and is required to attend case conferences and other meetings as directed. All client data must be accurate and up to date. Because the position manages confidential information for clients, the DH Case Manager must demonstrate the highest degree of professionalism. This position is full time.
Duties and Responsibilities<ul><li>
Provides support, assessment and case management service to individuals.
</li><li>Engage with residents and service providers through personal intervention/interactions, incorporating relevant information as part of care planning and referrals.</li><li>
Provide options/referrals related to substance use and mental health.
</li><li>Refers residents to appropriate community agencies for further assistance and provides discharge planning to ensure post-resident care.</li><li>
Provides outreach and information to the community regarding agency procedures and health and welfare agencies.</li><li>
Participates in intake planning including, but not limited to, intake calls and off-site interviews.</li><li>
Establish and monitor the care and growth plan for all residential clients.
</li><li>Work with intra-agency resources to maintain a clear and ongoing path of success for all residents.
</li><li>Provide outreach information to the community regarding housing graduates and success strategies proven by program participants.</li><li>
Provide linkages to agency, community and government services and monitor referral completion including state insurance continuation programs, medication assistance programs, benefits/health coverage programs such as, SSI, SSDI, Medicaid, Medicare, housing programs, etc.</li><li>
Maintain current documentation and records of case management services as well as statistical data required by the programs. Enter database information, and any other agency specific data in HMIS and other utilized agency programs in a timely manner.
</li><li>Assess client stability and provide referrals to increase the level of stability and self-sufficiency.
</li><li>Work closely with all departments to ensure client success.
</li><li>Assist in maintaining current information for client records and complete administrative reporting on a timely basis.
</li><li>Other duties as assigned.

Participate actively in any fund raisers provided by ASD.</li><li>
Respond to calls, and/or, emergencies at any time.</li></ul>
Qualifications

BA/BS required in social work, human services, or related field, MSW, LCSW, or three years of experience.</li><li>
Experience in psychology, social services and particularly in the field of HIV/AIDS care is highly desired.
</li><li>Preference will be given to those applicants who have experience providing case management to populations with issues including mental illness, substance abuse, chronic health and/or homelessness.</li><li>
Familiarity with databases, internet, Word, Excel, and Outlook</li><li>
Access to reliable transportation required</li><li>
Proficient in English oral and written language skills required</li><li>
Fluency in Spanish is highly desirable</li></ul>
Physical Requirements
Must be able to sit for long periods of time. Also, lift up to 15 lbs., stand, stoop, bend, push, pull and climb stairs.

The incumbent will participate in all agency training and development programs and will maintain continuing education requirements for continued certifications.

The Role
The PSH (Permanent Supportive Housing) Case Manager is responsible for ensuring the long-term residential stability of individuals experiencing homelessness through the Dallas County Voucher &amp; Housing Collaboration. Working in partnership with Housing Navigators and the Dallas County Housing Authority (DCHA), the Case Manager guides neighbors through every stage of the housing process&mdash;from initial engagement and eligibility confirmation through voucher application, housing search, lease-up, move-in, and ongoing tenancy support. This role promotes ASD&rsquo;s mission and goals, maintains the confidentiality of all clients, and upholds the dignity and integrity expected of a position of public trust.
What You&rsquo;ll Do
The PSH Case Manager focuses on delivering high-quality, trauma-informed case management services to neighbors who have experienced chronic homelessness and are enrolled in the DCHA voucher program. The Case Manager coordinates closely with Housing Navigators throughout all phases of the Dallas County Voucher &amp; Housing Collaboration workflow&mdash;from HMIS enrollment and document collection through voucher processing, unit approval, lease signing, move-in, and ongoing tenancy stabilization. This position is full-time and requires travel to housing sites, DCHA offices, and community locations.

</li><li>Knowledge of or experience with Housing Choice Voucher (HCV) programs, DCHA processes, or Permanent Supportive Housing models is strongly preferred.
</li><li>Experience working with populations affected by mental illness, substance use disorders, chronic health conditions, or HIV/AIDS is highly desired.
</li><li>Familiarity with HMIS and case management databases; proficiency in Microsoft Word, Excel, and Outlook.
</li><li>Familiarity with Padmission Journey or similar housing/case management platforms is a plus.
</li><li>Access to reliable transportation required; travel to DCHA offices, community sites, and client homes is expected.
</li><li>Fluency in a second language may be required.
