This role focuses on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization.</div>

<h3 class="jobSectionHeader"><b>Clinical Execution &amp; Patient Care</b></h3><ul>
<li>Counsel patients via telephonic outreach on medication adherence, address any medication related problems such as adherence barriers, polypharmacy, or high risk medication use, and make appropriate evidence-based recommendations to patients and Practice Care Teams</li>
<li>Perform focused clinical chart reviews as needed to inform patient interventions</li>
<li>Obtain and document patient commitment to medication adherence and address barriers in real time</li>
<li>Escalate complex clinical cases to prescribers or interdisciplinary teams as appropriate</li>
<li>Meet and sustain performance goals (e.g., STARs, HEDIS)</li>
</ul><h3 class="jobSectionHeader"><b>Clinical Innovation and Resource Optimization
</b></h3><ul><li>Collaborate with Physicians, clinical and administrative staff to optimize the Medication Management workflow and patient experience</li>
<li>Proactively identify gaps in medication management through patient interactions and translate patient insights into targeted recommendations that drive improvements in practice operations and clinical workflows</li>
<li>Participate in the design, implementation, and evaluation of pilot programs or process changes (&ldquo;test and learn&rdquo;) within the outreach model</li>
<li>Serve as a clinical subject matter expert, leveraging clinical pharmacy expertise to support organizational objectives and adapt to evolving healthcare needs</li>
<li>Develop and maintain standardized resources, educational tools, and clinical protocols to support improved medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patients</li>

</ul><h3 class="jobSectionHeader"><b>Minimum Qualifications:</b></h3><ul>
<li>Doctor of Pharmacy (PharmD)</li>
<li>Active, unrestricted pharmacist license in the state of practice; Valid multi-state licensure in at least two of the following states: AL, CA, DE, KY, LA, MS, NC, OH, PA, UT, VA, WV</li>
<li>Willingness to obtain and maintain additional licenses within 6 months of hire</li>
<li>Minimum of 3 years experience in value-based care, ambulatory care, or community pharmacy settings</li>
<li>In depth knowledge of pharmacotherapy and pharmacy practice</li>
<li>Strong verbal and written communication skills; Ability to translate complex clinical data into clear, understandable language for patients and care teams</li>
<li>Proficiency in telephony platforms and various Electronic Health Record (EHR) systems</li>
<li>Ability to work in a fast paced environment and adapt quickly to change</li>
<li>Ability to work between 8 AM - 7 PM EST as dictated by business needs</li>

Clinical Pharmacist, Pharmacy Outreach (Remote)

<div><div>The <b>Pharmacy Outreach Specialist</b> is responsible for patient outreach, coordinating with practices and pharmacies to promote medication adherence and improve patient health outcomes and pharmacy quality measures.</div>

Patient Outreach</b></p>

<li><p>Conduct &amp; handle high-volume outbound and inbound calls with patients, providers, and pharmacies using established scripts and protocols while meeting and sustaining performance goals (i.e. calls/hour, call quality, etc.)</p>

</ul><ul><li><p>Collaborate with patients to actively problem-solve, identify and remove barriers to medication adherence (i.e. cost, forgetfulness, or lack of perceived need) and/or escalate clinical barriers to a pharmacist</p>

<li><p>Overcome patient objectives and reluctance through influence, empathy, and education to secure patient commitment to medication adherence.</p>
</li>
<li><p>Proactively coordinate with medical providers or pharmacies as needed to ensure prescription refills occur as scheduled.</p>
</li>
<li><p>Accurately document all interactions, outcomes, and identified barriers in appropriate systems.</p>
</li>
</ul><p><b>Process Improvement, Learning &amp; Collaboration</b></p>

<li><p>Identify opportunities to enhance pharmacy workflows and processes</p>

Outreach Specialist, Pharmacy Outreach

<div><div><p>The Risk Platform team builds and runs Aledade's core risk and diagnosis platform: we own the Risk Engine that generates suspected diagnoses (PDx), performs HCC modeling, and calculates patient risk scores for Medicare and commercial contracts, aiming to make risk assessment accurate and efficient end-to-end.</p>
