Adult Outpatient Programs provide the least restrictive level of care where adults may receive clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer&rsquo;s presenting symptoms.</div>
Responsibilities:
<div>
Case Managers assist with monitoring consumer&rsquo;s well-being including needs in the areas of mental health, physical health, living, learning, working, and social environments. Case managers travel to wherever consumers reside, whether that is at home, boarding homes, or other community based living to provide monitoring, when working with children they would be going to the schools as well. The primary role of the case manager is to increase psychological adjustment for the consumer by empowering the consumers and sometimes family members with information, education and support in order to follow through once case management ends.</div><div></div><div><b><br>
Primary Job Functions:</b></div><div>
Clinical Responsibilities</div><ul><li>
Maintains caseload effectively, processing referrals in a timely manner and participates in assessment unit intake process as appropriate and requested.</li><li>
Conducts a (FNA) Functional Needs Assessment on each referral or identified patient</li><li>
Consults with Care Coordinator to explore any existing medical needs</li><li>
Comply with completing goals to be achieved, creates, and maintains an (ISP) Individualized Service Plan within 30 days of contact</li><li>
Provides appropriate linkage to community resources</li><li>
Coordinates implementation of internal services as appropriate</li><li>
Maintains an itemized list of resources being utilized for each patient and regularly monitors to ensure the service is still needed and/or appropriately meeting their needs</li><li>
Collaborates with Benefits Specialist, Representative payee, Transporter/Shopper, (BLS) Basic Living Skills instructors, etc</li><li>
Effectively collaborates with the treatment team members and regularly provides feedback on the patient&rsquo;s status</li><li>
Evaluates the patient for discharge planning</li><li>

Physical Requirements</b></div><ul><li>
Frequent standing, sitting, and walking which are in bursts of 5 to 10 minutes.</li><li>
There are times when this can be extended to pacing with the consumer (time</li><li>
could be undetermined) and at times sitting and standing can be extended with the</li><li>
employee having the ability to change positions as permitted by the observance</li><li>
and behavior of the consumer at that time.</li><li>
Occasional forward bending from the hips while kneeling to reach to the side.</li><li>
Occasional need to be able to assume a full squat/lunge from a standing position.</li><li>
Occasional need to maintain back extension while being on stomach with one&rsquo;s</li><li>
own body weight on hips and legs. This position is maintained for several</li><li>
minutes.</li><li>
Occasionally, be able to put one&rsquo;s own body weight on one knee and one elbow</li><li>
on the same side of one&rsquo;s body.</li><li>
Occasionally, be able to kneel and reach to same side toward the floor.</li><li>
Lift up to 10 pounds occasionally during shift.</li><li>
Strong grasp is needed to use keys to open doors throughout the facility, and to</li><li>
maintain constant contact with a consumer during a take down procedure or with</li><li>
other hold techniques.</li><li>
Pushing/pulling force up to 25 pounds occasionally during a shift.</li></ul>
Qualifications:
<div>
Bachelor&rsquo;s degree in a mental health discipline. Must be at least age 23, maintain a valid verifiable driver license and maintain a clear driving record.</div><div>
Knowledge in mental health service provision, outlined below, to be attained within six (6) months of employment:</div><ul><li>
Knowledge of psychiatric disorders in the adult population</li><li>
Knowledge of developmental disabilities and substance use in the adult population</li><li>
Knowledge of cultural diversity</li><li>
Knowledge of behavioral management and crisis intervention</li><li>
Knowledge of available community resources and internal services</li><li>
Certified by the Alabama State Department of Mental Health to provide adult case management services</li></ul></div>

Case Manager

<div>Responsibilities:
<div>
Nurses in the hospital setting are responsible for completing the admission process for all consumers entering the facility, assigning/supervising routine task assignments of unit staff, administering medication as needed, communicating daily with the physician regarding the consumer&rsquo;s physical and mental status, attend multidisciplinary meetings as held, and deescalating or resolving crisis situations on the unit.</div><div></div><div><b><br>
Essential Functions</b></div><ul><li>
Completion of the admission process for consumers including: Nursing Assessments, Medical Checklist/Physical Health/Nutritional screening, Medication List, Medication Administration Record, vital signs, height/weight, transcribing physician orders, AIMS, and admission notes.
</li><li>Observes consumers behavior in groups and activities, documents observations in consumer records, interacts with consumers and evaluates needs to ensure that proper information is reported from shift to shift and during staffing with the Treatment Team.</li><li>
Supervises nursing care overseeing routine task assignments of RNs, LPNs, CNAs, and Behavioral Aides/Specialists</li><li>
Administers medications, observing the Six Rights of medication administration</li><li>
Provides medication education for the consumer and family, including uses and potential side effects. Monitors consumers for potential side effects of medications and reports findings to the physician</li><li>
Communicates effectively with the physician regarding the consumer&rsquo;s physical and mental condition</li><li>
Interfaces with the multi-disciplinary team and actively and cooperatively participates in development of the interdisciplinary treatment plan</li><li>
Performs venipuncture for laboratory requests, monitor results, and reports all abnormal findings to the physician</li><li>
Provides emergency care as needed (CPR, MindSet, PRN medications), according to AltaPointe Policy &amp; Procedures</li><li>
Assists consumers with personal care issues (bathing, toileting and other personal hygiene items)</li><li>
Demonstrates willingness to work on any unit at EastPointe or BayPointe based on facility needs and as directed by the supervisor</li><li>

Pushing/pulling force up to 25 pounds occasionally during shift.</li><li>
There are times that require the employee to be able to ambulate as quickly as possible to assist coworkers with a consumer in a restraint procedure. (Example: jog/run up to 0.1 mi).</li><li>
At times therapy may be inside or outside the facility; therefore, some activities will dictate physical requirements for playing or games.</li></ul><br>

Facilitate PA&rsquo;s for prescriptions</li><li>
Updates patient medical records as needed</li><li>
Assists in arranging hospital admissions and laboratory services

</li><li>May assist instructing patients about medication and special diets
</li><li>Authorizes prescription refills as directed

Researches all patients that present with no insurance through Medicaid and Medicare eligibility sites.</li><li>
Verifies all insurance eligibility and enters accurate information into Avatar.</li><li>
Accurately collects &ldquo;out-of-pocket&rdquo; costs (i.e. co-pays, deductibles and co-insurance).</li><li>
Balances out cash box/drawer accurately (daily) and prepares deposit.</li><li>
Makes copies, faxes documents and performs other related clerical duties as needed.</li><li>
Adheres to professional code of ethics and to all 5 Star Customer Service Guidelines when dealing with Practice Manager, CRNP/Physician, Team Leader, co-workers and patients</li></ul><p></p><div><br>
Additional job duties depending on location may be as follows:</div><ul><li>
Distributes prescriptions from the doctor/CRNP/PA to the consumer.
</li><li>Assists with the patient assistance pharmacy forms as needed, which includes communicating with the consumer regarding proof of income.

</li><li>Coordinates appropriate services for consumers in crisis.
</li><li>Distributes Daily DSR reports.

Possesses current certification for a <b>Certified Clinical Medical Assistant</b>. Must be able to stand for long periods of time, continuously bend, stoop, kneel, crouch, reach and lift at least 10 pounds. Must have visual acuity in at least one eye and manual dexterity. Must have the ability to communicate verbally, withstand continual deadlines, concentrate and maintain accuracy in spite of frequent interruptions.</div><div></div><div><br>
