What You&rsquo;ll Do<ul><li>
Evaluate residents and develop individualized treatment plans in collaboration with residents, families, and the interdisciplinary care team</li><li>
Provide physical therapy services focused on restoring mobility, strength, function, and independence</li><li>
Conduct screenings and identify residents who would benefit from intervention</li><li>
Supervise and collaborate with PTA staff as applicable</li><li>
Initiate referrals when appropriate in accordance with department procedures</li><li>
Document clearly and efficiently in support of quality outcomes</li></ul>
What We&rsquo;re Looking For<ul><li>
Bachelor&rsquo;s, Master&rsquo;s, or Doctorate in Physical Therapy from an accredited program</li><li>
Current PT license or license-eligible</li><li>
Commitment to patient-first, outcomes-driven care</li><li>
Team-oriented mindset with strong communication skills</li><li>
