Summary:
The SUD Case Manager provides comprehensive support and coordination of care for clients facing substance use challenges. This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services. The Case Manager is dedicated to facilitating clients' recovery and promoting their overall well-being and independence. 
Duties and Responsibilities:<ul><li> Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.</li><li> Manage case assignments, draft service plans, and review case progress regularly.</li><li> Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.</li><li> Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.</li><li> Facilitate clients in achieving personal independence and assist in developing daily living skills.</li><li> Perform other duties as assigned.</li><li> Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.</li></ul> 
Supervision Given:
This position does not supervise any staff. 
Supervision Received: 
The Behavioral Health Case Manager reports to their assigned Operations Managers. 
Qualifications:<ul><li> Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.</li><li> Knowledge of behavioral health practices and community resources.</li><li> Ability to work independently and as part of a team.</li></ul> 
Education:<ul><li> High school diploma or equivalent is required;</li><li> a college degree in Human Services is preferred.</li></ul> 
Certification:
 &middot; CDCA licensure<ul><li> NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided).</li></ul>
 &middot; CPR certification is preferred.
Experience:<ul><li> Minimum of 3 years in behavioral health or a related field is preferred, but not mandatory.</li></ul> 
Physical Effort:<ul><li> Requires sitting for extended periods and occasional lifting of up to 20 pounds.</li></ul> 
Background:<ul><li> Must pass all Federal and State background checks according to company policy.</li></ul>
Schedule:<ul><li> Flexible scheduling options allow for accommodating personal availability, enabling as little or as much work as desired.</li></ul>
