Under the direction of the department Director and/or designee, performs a wide variety of clerical tasks, technical duties, and transportation for all radiology modalities requiring a high degree of accuracy with latitude for independent initiative and judgment. Promotes, follows, and adheres to the values set by AVH. Delivers quality customer service which meets or exceeds expectations to patients, physician office staff, and other customers.

Duties and Responsibilities:

A. Technical Support
<ul><li>
Assist technologist with moving patients on and off the exam table within employee&rsquo;s own bodily implant restrictions relative to radiology, e.g. AICDs, pacemakers, etc.</li><li>
Transport patients between both hospital areas and all radiology modalities.</li><li>
Support and assist technologist with daily patient work flow.</li><li>
Review and verify in-conjunction with technologist on patient orders, patient history, and lab work pertaining to specific exam.</li><li>
Assist technologist and radiologist, as needed.</li><li>
Report any equipment malfunction to supervisor or manager.</li><li>
Prepare exam rooms for upcoming exam.</li><li>
Work in any area and flexible in meeting the needs of the department.</li><li>
Completes exams in RIS and checks PACS to insure exam has crossed into the system.</li><li>
Clean and disinfect exam rooms, as needed.</li><li>
Observe safety measures as they pertain to modalities assigned, radiation protection and MRI safety.</li><li>
Assist with stockroom responsibilities ensuring adequate supply inventories.</li><li>
Follows established department procedures to accurately verify and transport the proper patient by visually checking patient identification or verifying with nursing staff.</li><li>
Adherence to the Patient Identification Policy.</li></ul>

B. Clerical Support
<ul><li>
Assist in obtaining patient signature on validated consent forms.</li><li>
 Obtains authorization for release of radiology records.</li><li>
 Burn CD&rsquo;s, print films and digitize, as needed.</li></ul>

C. Telephone Support
<ul><li>
Answers telephone in compliance with the hospital telephone etiquette.</li><li>
Document accurate messages and/or route calls to the appropriate parties.</li></ul>

Under the direction of the Administrator, to provide accurate maintenance of operational and clinical data relating to patient medical records. Maintain a cost effective inventory system of departmental medical supplies. 


Essential Duties and Responsibilities:

A. Records Coordination</h1>

1. Creation and tracking of department patient master charts.

2. Accurate filing in patient medical record of all legal patient documentation including but not limited to consents, clinical notes, physician orders, lab reports in a chronological order by discipline

3. Coordination of medical records requests and/or legal subpoenas.

4. Communicates regularly with practitioners regarding incomplete charts due to unsigned orders.

5. Runs incomplete chart reports as defined by procedure.

6. Provides access as needed to the patient master chart to field staff.

7. Reviews each discharged patient&rsquo;s chart for incomplete or missing items and identifies the responsible practitioner or discipline for each of these items.

8. Replenish forms, packets and field charts for clinical staff as needed.

B. Inventory Systems

1. Maintains adequate and organized medical supply inventory for department staff.

2. Responsible for requisition submission, stocking and monitoring usage of medical supplies for quality control and cost effectiveness.

<div>Job Objective: To provide, supervise and coordinate skilled nursing care, personal care and other services given to her/his assigned patients. The Registered Nurse (RN) performs the comprehensive patient assessment, develops plan of care, oversees the execution of plan of care, ensures care is provided as ordered in the plan of care, delegates nursing services to Home Health Aides (CHHA) and Licensed Vocations Nurses (LVN) and provides skilled nursing care. 

Duties and Responsibilities:

A. Patient Care
1. Acts as the patients advocate and protects patients&rsquo; confidentiality.

3. Demonstrates knowledge regarding special needs and behaviors of age specific groups and demonstrates proficiency in age specific competencies.

4. Demonstrates knowledge of medical disease processes, surgical procedures, psychosocial behavior, and nursing care theory and practice.

5. Provides the initial nursing assessment on patients involving physical, physiological and psychosocial status. This assessment should include therapeutic regimen and environmental factors involved in the patient&rsquo;s care.

6. Performs patient care according to plan of care and MD orders.
7. Counsels and instructs patients and caregivers in meeting nursing and related needs.

8. Coordinates care with physicians and other disciplines involved through collaboration with other agency staff and participation in case conferences.

9. Encourages and moves patients/caregivers towards self-management of patient&rsquo;s care.

10. Initiates a plan of treatment and makes necessary revisions based on nursing assessment. The plan of care is established based on collaboration with patients, caregivers and physicians. Establishes appropriate goals and discharge plan. Established and revised plan of treatment in accordance with physician orders. Routinely re-evaluates plans of treatment and updates as needed.

11. Utilizes Best Practices as available in all aspects of patient care.

12. Utilizes Disease Management programs once established.

B. Communication

2. Responsible for Supervision of LVN by overseeing patients&rsquo; care, communication with LVN, updating the plan of care as needed, performing and documenting LVN supervisory visits every 30 days.

<div>Job Objective:

Under the direction of the Cardiovascular Services Director manages, develops and coordinates the Cardiovascular Special Programs, directs and supervises all functions and activities, develops policy/procedures/protocols to meet standards and regulations as appropriate. Collects/supervises and monitors data collection, process and outcomes of Trans-catheter Valve Replacement and Left Atrial Appendage Occlusion Procedures and other special programs including but not limited to the ECMO program. The Cardiovascular Special Programs Coordinator facilitates the professional development and education of nursing staff and other nursing team members. The Cardiovascular Special Programs Coordinator works in close collaboration with the Cardiovascular Services Director and Medical Director of the Cardiovascular Program and all multidisciplinary team members to improve the outcomes of the Cardiovascular Patient.

The Cardiovascular Special Programs Coordinator is responsible for program/project management, competency assessment, education, measurement and evaluation including cost/benefit analysis, implementation of evidence-based practice, working with accrediting bodies such as The Joint Commission and development and coordination of excellence initiatives. The Cardiovascular Special Programs Coordinator practices in accordance with American Nurses Association (ANA) Scope and Standards of Practice for Nursing Professional Development, Scope, and Standards of his/her professional specialty.
