Qualification: Master&rsquo;s degree in Social Work (MSW)Transplant CoordinatorKey Responsibilities:<ul><li>Patient Coordination:</li><li>Manage the care of transplant candidates, recipients, and donors throughout the continuum.</li><li>Conduct patient assessments, education, and counseling.</li><li>Coordinate patient testing, diagnostics, and consults.</li><li>Clinical Duties:</li><li>Monitor and document patient status, progress, and complications.</li><li>Participate in organ offer evaluations and donor-recipient matching.</li><li>Coordinate transplant surgery logistics with surgical and medical teams.</li><li>Administrative Tasks:</li><li>Maintain accurate patient records in accordance with hospital, state, and national guidelines (NOTTO).</li><li>Assist with data collection and reporting to regulatory bodies.</li><li>Participate in quality assurance and performance improvement programs.</li><li>Communication and Collaboration:</li><li>Liaise with physicians, nurses, social workers, and other healthcare staff.</li><li>Serve as a point of contact for patients and families.</li></ul>Medical Social WorkerKey Responsibilities:<ul><li>Psychosocial Assessment:</li><li>Evaluate patient and family psychosocial functioning, support systems, coping skills, and financial readiness for transplant.</li><li>Provide recommendations to the transplant team regarding suitability and potential barriers.</li><li>Counseling &amp; Support:</li><li>Offer individual and family counseling related to stress, anxiety, grief, and adjustment.</li><li>Provide crisis intervention as needed.</li><li>Resource Coordination:</li><li>Help patients access financial aid, housing, transportation, insurance, and other community resources.</li><li>Collaborate with financial counselors and patient navigators.</li><li>Patient Education:</li><li>Educate patients and families on the psychosocial aspects of transplantation, including lifestyle changes, compliance, and support networks.</li><li>Interdisciplinary Collaboration:</li><li>Participate in multidisciplinary meetings, case conferences, and discharge planning.</li><li>Advocate for patient needs within the healthcare system.</li></ul>Job Type: Full-time
