Job Summary
 Core to the ArchWell Health approach is to have a robust clinical care team focused on the needs of our members. Care teams are responsible for delivering excellent and compassionate primary care and coordinating the care of our members with the broader healthcare delivery system. The Medical Assistant – Internal Float (MA-Float) is a critical role to flexibly support our members and their families from the moment they arrive to the moment they leave the centers. The MA-Float may be asked to serve in front office or direct patient care activities, depending on center and member needs. This role supports the PCP day-to-day and ensure efficiency in the care provided to patients. MAs work closely and collaboratively with their teammates to greet, schedule, and room ArchWell Health members. S/he will be expected to assist in assessing patient’s health conditions, including performing routine diagnostic testing during appointments. The MA-Float may be asked to provide coverage outside of their primary center.
 
Duties/Responsibilities
 <ul>
<li>Welcome members upon arrival and help them feel safe and at home, navigating through a seamless and clear check-in process</li>
<li>Schedule appointments and coordinate referrals for a multi-disciplinary care team</li>
<li>Update member information in the electronic medical records system</li>
<li>Request medical records and upload documentation to electronic medical records system</li>
<li>Conduct intake with patients, measuring and recording vital signs, patient interview, and medical history; perform examinations and routine screenings, ensuring patients are informed and all questions and concerns are addressed for each procedure</li>
<li>Critically listen and engage with patients throughout appointment, ensuring needs and/or changing in patient conditions are reported to PCP, specialists, and/or other clinical personnel</li>
<li>Perform all tasks with a hyper focus on patient experience, including answering of incoming calls and processing of patient phone messages, including returning calls; routing calls to other team members as appropriate; and calling patients to obtain and relay pertinent information for the physician</li>
<li>Change dressings, apply bandages, remove sutures and other first aid procedures; use CPR skills when necessary, all under physician supervision</li>
<li>Ensure compliance with OSHA safety standards and conduct accurate, legal, and ethical documentation at all times</li>
<li>Assist with center events, as needed</li>

<li>A minimum of 1 year work experience as a Medical Assistant preferred, but not required</li>
<li>6+ months phlebotomy experience is highly desired</li>
<li>BLS for Healthcare Providers required</li>
<li>EClinicalWorks or similar EMR experience</li>
<li>Experience working with geriatric patients is a plus</li>

The Care Coordinator is responsible for supporting our members as they navigate the complex healthcare delivery system, ensuring every member receives the right care in the right setting. This individual will serve as a guide for the member and their families to ensure the member meets with the right specialists, inside or outside the center, in a timely manner. An ideal candidate will have a deep understanding of the healthcare ecosystem, have strong attention to detail, and be driven by a passion for providing quality care to seniors.
 
Duties/Responsibilities:

<li>Partner with ArchWell Health's multi-disciplinary care team to gain a deep understanding of each member's care needs, including identifying gaps and barriers to treatment</li>
<li>Coordinate referrals to external providers, vendors, and services to ensure care needs are met</li>
<li>Complete proper documentation for member referrals to ensure records are accurate and current</li>
<li>Build and nurture relationships with members and their families to successfully facilitate care coordination</li>
<li>Carry out all activities and provide ongoing monitoring as outlined in a member's care plan to ensure routine follow up and progression is made on care plan goals</li>
<li>Adhere to HIPAA standards and confidentiality of protected health information (PHI)</li>
<li>Promote clear communication among care team and treating clinicians to ensure positive member outcomes</li>
<li>Assist members in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).</li>
<li>Be the system navigator and point of contact for members and families, with patients and families having direct access for asking questions and raising concerns.</li>

Reporting to the Clinical Pharmacist, the Pharmacy Technician will be providing direct patient care and collaborating with other healthcare team members in a virtual environment to optimize the pharmaceutical care for our patients. This individual will conduct patient outreaches per existing protocols and assist patients with navigating the medication use aspects, including reviewing components of their plan benefits.  Conduct telephonic outreaches and answer incoming calls regarding patient&rsquo;s medication use, including but not limited to medication adherence, and assist with refilling and following-through in a prompt and respectful manner
