Responsibilities:
<ul><li>Provide holistic nursing care to patients in a correctional setting.</li><li>
Assist health care providers, registered nurses, and allied health professionals with examinations, treatments, special tests, and routine procedures.</li><li>
Obtain patient health history, laboratory specimens, vital signs and administers TB skin tests.</li><li>
Conduct lab tests and ensures the results are delivered to the correct physician or registered nurse.</li><li>
Ensure patient is receiving proper care for injury or condition.</li><li>
Review patient chart; records and reports observed symptoms, reactions, treatments, and changes in patient's conditions.</li><li>
Coordinate patient transfers to medical facilities or outside health care providers; administer medications; implement healthcare practitioners' orders; and document all patient contacts using the POMR format.</li></ul>
Qualifications:
<ul><li>Current/Active state license as an LPN</li><li>
Current CPR certificate</li><li>
Proficient with electronic medical records</li><li>
Ability to pass pre-employment Level 2 clearance to include a background check and drug screen</li></ul><div></div><div><br>

Responsibilities:
<div>
Monitors chronic care logs and caseload as needed to supplement referrals by staff.</div><div>
Develops discharge planning referral criteria for intake medical screeners.</div><div>
Consults routinely with medical providers on current caseload to identify patients meeting criteria for receiving pre-release health planning</div><div>
Consults with the Armor behavioral health provider (including psychiatrist and licensed mental health professionals) on specific patients with complex needs that require supplemental coordination in addition to individual treatment plans or behavioral health referral plans upon release from custody</div><div>
Participates in multidisciplinary planning in support specialized transitional programs dedicated to seriously or chronically ill patients (such as community based mental health teams and HIV Jail Linkage programs)</div>
Qualifications:
<ul><li>Minimum of two years&rsquo; experience, preferably in correctional health or criminal justice environment as case manager; discharge planner; nurse performing similar tasks related to referral, case management, and/or care coordination; or mental health related service coordinator</li><li>
Bachelors&rsquo; Degree in social sciences, human services, or health care related field or Nursing Degree</li><li>
Excellent written and verbal communication skills; excellent computer skills</li><li>
Familiar with health records, medical terminology, and health information services</li></ul><p>
