Regional Case Manager SpecialistLocated in the heart of scenic, rural Western Pennsylvania, Armstrong County CommunityAction Agency, d/b/a, Armstrong County Community Action Partnership (ACCAP), is a501(c)(3) nonprofit agency incorporated in 1965. We are a community-based, grassrootsorganization serving individuals facing economic hardships. Our mission is to empowerindividuals and families in Armstrong County and surrounding areas through resourcemobilization, sustainable solutions, and support thus fostering self-sufficiency and community strength.Our agency is seeking a talented individual to join our Housing team as a full-time RegionalCase Manager Specialist. The following is an outline of the necessary tasks and duties:DUTIES AND RESPONSIBILITIES:<ul><li>Position is based in Armstrong County (Kittanning) with intermittent travel to Butler and Indiana counties.</li><li>Responsible for developing an Individualized Service Plan (ISP) with the goal of assisting agency clients in obtaining the necessary supportive services to promote self-sufficiency.</li><li>Submit ISP plan for review and approval to direct supervisor.</li><li>Periodically review ISP to insure goals are being addressed and met.</li><li>Maintain regular and consistent communications with clients/potential clients in a timely and professional manner including initiating home visits as needed.</li><li>Write compressive and accurate case notes in a consistent and timely manner.</li><li>Maintain accurate client files and records according to specified funding sources, department regulations, and assigned caseload.</li><li>Insure delivery of services in a timely manner.</li><li>Provide budget and family finance management counseling.</li><li>Attend, and participate in, regular case management meetings with the Housing Department Manager.</li><li>Coordinate activities relating to clients with other social service agencies.</li><li>Insure client tracking activities are properly carried out as required by the Housing</li><li>Department Manager in order to assess the self-sufficiency of the program participants</li><li>following the client&rsquo;s release from the program.</li><li>Complete direct client services to the homeless, near homeless, or low-income residents</li><li>involved in any housing or utility assistance, as well as any emergency food and shelter</li><li>programs on behalf of the agency.</li><li>Complete and enter client data correctly into various database platforms.</li><li>Complete transmittals and vouchers pertaining to client services.</li><li>Network with the appropriate social service agencies, utility companies, landlords, real</li><li>estate agencies, or any other group relating to client assistance in order to enhance</li><li>coordination efforts and the delivery of services to agency clients.</li><li>Keep comprehensive and accurate records of client statistics and assist in preparation of all appropriate reports, including quarterly and year-end reports, for submission to the respective funding sources. All documents must be submitted to the Housing Department</li><li>Manager for review and approval prior to submission.</li><li>Participate in the Homeless/Housing Advisory Committee meetings to share information</li><li>about homeless, near homeless, and low income clients, as required.</li><li>Participate in the yearly program evaluation process.</li><li>Attend meetings as required by the Executive Director, Housing Department Manager, or his/her designees.</li><li>Search and locate rental apartments for clients using all available resources including</li><li>phone calls, emails, internet, newspapers, etc.</li><li>Perform rental inspections.</li><li>Coordinate with clients and landlord in obtaining signatures in leases.</li><li>Assist with other Housing Department tasks as needed.</li><li>Travel to on-site locations appropriate to duties and responsibilities when necessary, and is instructed by the Executive Director, Housing Department Manager, or his/her designees.</li><li>Abide by the agency policy that contacting state or federal funding sources is prohibited unless instructed by the Executive Director, Housing Department Manager, or his/her designees.</li><li>Complete any and all duties assigned by the Executive Director, Housing Department Manager or his/her designees.</li></ul>The successful candidate will possess the following qualifications:<ul><li>Must have a minimum of a high school diploma, or its equivalent (GED); Associates Degree or post-secondary education preferred (but not required) in psychology, sociology, counseling, education, communications, or related discipline.</li><li>A minimum of one year related experience and program management experienced preferred, but not necessary.</li><li>Computer literate and possess aptitude for learning new software programs</li><li>Effective communications skills.</li><li>Possess excellent organizational and problem-solving skills.</li><li>Ability to ensure the successful creation and completion of departmental goals.</li><li>Identify and resolve problems and accomplish goals.</li><li>Demonstrate the ability to work independently with little or no supervision.</li><li>Demonstrate the ability to work as a team.</li><li>Familiarity with Microsoft Office and Excel helpful.</li><li>Able to operate basic office equipment such as telephone, copy and fax machines, and postage machine.</li><li>Ability to successfully coordinate with County and State agencies.</li><li>Must be empathetic with regards to the needs of the low-income and economically disadvantaged.</li><li>Must be able to lift 20 pounds.</li><li>Must be able to sit at desk to complete reports, attend Zoom trainings, etc.</li><li>Other duties as assigned.</li><li>PA State Criminal Background Check, PA Child Abuse History Clearance, FBI Fingerprint Criminal Records Check, and Drug Testing Required.</li><li>Valid driver's license and good driving record required.</li><li>Personal vehicle or access to independent transportation required.</li><li>Proof of motor vehicle insurance (for use of personal vehicle).</li><li>Must adhere to agency confidentially standards at all time.</li><li>Social services experienced preferred, but not necessary.</li><li>Professional appearance; positive attitude.</li><li>Adhere to the agency&rsquo;s Confidentiality Policy.</li></ul>Equal Opportunity Employer:Armstrong County Community Action Partnership is an equal opportunity employer.Full-Time Benefits include: UPMC Health Insurance; Principal Vision/DentalInsurance; 14 observed holidays; vacation/sick/personal days; 403(b) pension plan;short-term disability; possible remote work opportunities; casual Fridays; fun monthlystaff lunches; time off for Jury Duty; advancement opportunities.Salary begins at $15.00 / hour.Please email Cover Letter and Resume to:Paige Hockenberry, Housing Department Manager, at Paigeh@armstrongcap.comOR mail to:Armstrong County Community Actionc/o Paige Hockenberry, Housing Dept.
