<div>We are hiring for an RN to do on call Friday - Saturday &ndash; Sunday for our Hospice Team in Phoenix! 
 
This is an On-Call Position for Weekends and Evenings. 
 
On-Call Rates: 
 $50 On-Call Rate for P.M.s weekday and $100 for Sat/Sun's.
<ul><li>RN On Call Eval - $195.00 per visit plus $10.00 car visit for mileage/phone allowance</li>
<li>RN On Call Routine - $105.00 per visit plus $10.00 car visit for mileage/phone allowance</li></ul>

What you will do:
<ul><li>Apply specific criteria for admission and re-certification to establish patient&rsquo;s eligibility and appropriate levels of care.</li>
<li>Assess the impact of the serious illness or terminal diagnosis on patient&rsquo;s physical, functional, environmental, safety, ADL and psycho-social needs, including risk for pathological grief.</li>
<li>Assess all aspects of the patient&rsquo;s pain and develop an individualized pain management plan.</li>
<li>Identify patient verbal and non-verbal communications patterns.</li>
<li>Assess the ability of the caregiver to meet the patient&rsquo;s immediate needs at start of care and throughout care.</li>
<li>Develop an individualized palliative plan of care based on the needs identified which demonstrates management of discomfort and provision of symptom relief.</li>

</ul>Who you are:
<ul><li>Graduate of an approved school of professional nursing.</li>
<li>Current, active license in the state of Arizona Board of Nursing,</li>
<li>Current CPR, health clearance, TB clearance.</li></ul></div>
