Program Description:
The Auberle Housing Programs work with individuals and families to obtain and maintain long-term housing solutions through case management, the development of life skills, and connection to resources.
Position Title and Overview: Housing Case Manager
<ul><li>Engage with individuals and families to support them in achieving their housing goals and addressing their housing needs.
</li><li>Meet individuals where they are, both physically and emotionally, to conduct intake assessments and provide case management services.
</li><li>Travel throughout Allegheny County in your vehicle with program participants to locate housing units suitable to meet their housing needs.
</li><li>Conducting case management meetings in participant units
</li><li>Evaluate participant eligibility monthly.
</li><li>Engage and build relationships with landlords and provide mediation services as needed.
</li><li>Develop relationships with community resources and connect program participants as applicable for services such as employment, education/ training, transportation, childcare, food and healthcare.
</li></ul>Location and Travel Requirements:
This position is primarily located in McKeesport. All positions are also expected to come to training, meetings, and other events at Main Campus in McKeesport, as scheduled.
The travel required for this position is frequent.
Position Requirements:
<ul><li>Act 33/34 and FBI Clearances required.
</li><li>Bachelor's degree preferred, HS diploma/GED accepted.
</li><li>Valid PA driver&rsquo;s license, vehicle, registration, and proof of insurance.
</li><li>Involves exposure to tobacco smoke, domestic animals, household pests, and other conditions common to domestic environments.
</li><li>Requires working varied hours, including evenings, some weekends and holidays.
</li><li>Flexibility to work at either location as program needs change. (FES ADDED)

Program Description:
Foster Care Program is located at main campus in McKeesport with work throughout Allegheny and neighboring counties where resource homes are located and children &amp; young people served are from. Work in this program plays a crucial role in the lives of children and families, offering support and ensuring the best possible outcomes for all parties involved. Foster care programs work diligently to make sure that children in the foster care system receive the care, stability, and guidance they need to thrive. This work is more than just a job; it&rsquo;s a calling to make a profound impact on the lives of vulnerable children and families. This pivotal role involves advocating for the well-being of children placed in resource homes, working closely with natural and resource families to create a safe, nurturing, and supportive environment. As a foster care worker, you will have the opportunity to guide and support children through challenging transitions, ensuring they receive the care, stability, and opportunities they need to thrive.
Position Title and Overview: Case Manager
<ul><li>Connect, develop, and maintain a strong, trusting relationship with the families served and children &amp; young people placed to foster engagement and support growth.
</li><li>Conduct regular home visits to monitor the child&rsquo;s progress and well-being
</li><li>Develop and implement individualized care plans.
</li><li>Coordinate with natural families, resource families, and other service providers to promote open communication, collaboration, and to ensure comprehensive care for the children.
</li><li>Conduct regular meetings with children &amp; young people, caregivers, and professionals in a variety of settings- homes, community spaces, professional offices.
</li><li>Maintain detailed, accurate, and timely documentation to ensure safety and provide effective tracking and reporting.
</li><li>Provide on-call coverage assisting families through crisis intervention transportation.
</li></ul>Location and Travel Requirements:

 Program Description
 
 
 
 The 412 Youth Zone is located in downtown Pittsburgh. The program provides comprehensive, welcoming, and youth driven services and support to young adults transitioning out of the foster care system or who are experiencing unstable housing.
 
 
 
 Position Title And Overview
 
 
 
 Young Adult Case Manager
 
 
 <ul>

 Develop and maintain strong, trusting relationships with young people to foster engagement and support their individual growth.

 Conduct regular meetings with young people in their homes, community settings, or at the

 Collaborate with community partners and service providers to coordinate care, maximize support, and ensure young adults have access to necessary resources.

 Facilitate and promote various programs occurring at the drop-in center to engage young people and meet their diverse needs.

 **Location and Travel Requirements:**
 
 
 This position is primarily located in Downtown Pittsburgh. All positions are also expected to come to training, meetings, and other events at Main Campus in McKeesport, as scheduled.
 
 
 The travel required for this position is regular.
 
 
 This position requires frequent travel within the community, including visits to young people's homes, meeting in community settings, and transporting young people to appointments or other essential destinations.
 
 
 
 Position Requirements

 Act 33/34 and FBI Clearances required.
 </li>
 <li>
 Bachelor's degree preferred, HS diploma/GED required with experience working with youth. Special consideration will be given to individuals with previous DHS system involvement as a client. Other combinations of education and experience will be considered.

 <li>
 Valid PA driver's license, vehicle, registration, and proof of insurance.
 </li>
 <li>
 Requires travel to youth homes and throughout the community.
 </li>
 <li>
 Involves exposure to tobacco smoke, domestic animals, household pests, and other conditions common to domestic environments.

 Requires working varied hours, including evenings, some weekends and holidays.
