<div>Join our compassionate team as an LPN/RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. This position is part-time weekends, particularly Sunday. Open for PRN throughout the week as well.
What You'll Do:<ul><li>
Manage patients, ensuring they receive appropriate care.</li><li>
Complete assessments, create care plans, and document care.</li><li>
Collaborate with doctors, social workers, and other team members.</li><li>
Provide support and education to families.</li><li>
Enjoy flexible scheduling and work-life balance (avoid burnout!).</li></ul> 
We're Looking For:<ul><li>
Idaho RN/LPN License.</li><li>
2+ years of experience (pref. hospice).</li><li>
Strong communication and teamwork skills.</li><li>
Valid driver's license &amp; reliable transportation.</li></ul> 
Certifications, Licenses and Registrations<ul><li>
Registered Nurse licensed in the state of employment.</li><li>
Minimum of one (1) year nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years is preferred.</li><li>
Certification in Hospice and Palliative Nursing a plus but not required.</li><li>
Current CPR certification.</li><li>
Must have a valid driver&rsquo;s license and auto liability insurance.</li></ul> 
Other Skills and Abilities<ul><li>
Understanding and acceptance of hospice philosophy.</li><li>
Strong clinical, organizational and interpersonal skills.</li><li>
Strong written and oral communication skills.</li></ul>
