 Qualifications:
 
 
 
 
 CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

 Current, active Registered Nurse (RN) license in North Carolina (required).
 </li>

 Bachelor&rsquo;s degree in Nursing (BSN) preferred.

 Minimum of 2 years of experience in nursing, with at least 1 year in a home health or community health setting preferred.
 </li>

 Experience in leading or coordinating patient care is strongly preferred.

 Skills and Abilities:
 </li>
 <ul>
 <li>
 Strong clinical assessment and critical thinking skills.
 </li>
 <li>
 Excellent leadership, communication, and interpersonal skills.
 </li>
 <li>
 Ability to work independently while managing a caseload of home health patients.
 </li>
 <li>
 Knowledge of home health care regulations, including Medicare and Medicaid guidelines.
 </li>
 <li>
 Proficiency with electronic health records (EHR) and other healthcare software.
 </li>
 <li>
 Strong organizational and time management skills, with the ability to prioritize patient needs.
 </li>
 <li>
 Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

 Job Summary
 
 
 
 Evaluate the clinical needs of newly referred and hospitalized hospice patients. Identify and inform patient and family about appropriate resources. Work with the patient/caregiver and appropriate providers to establish patient/caregiver care goals and identify and implement interventions.
 
 
 With attending physician and selected members of the IDT, ensure that a medical plan of care is established and implemented for the patient. Communicate effectively with other members of the interdisciplinary team and health care providers, as needed.
 
 
 
 Community And Hospital Staff Relations

 Collaborates with hospital staff and Palliative Medicine Team to ensure effective and efficient working operations for referrals and current hospice patients.

 Must be currently licensed as an RN in North Carolina with a minimum of 3-5 years of experience in an acute or home health setting.
 </li>
 <li>
 Bachelor's degree in nursing and Hospice experience preferred. RN Certification in Hospice and Palliative Care preferred.

 Understand and be committed to the Hospice and Palliative philosophy of care.

RN Hospital Nurse Liaison

 Nurse Practitioner
 
 (Home Based Primary Care). This is a
 
 full-time
 
 position;
 
 Monday - Friday 8:00am - 5:00pm
 
 (office base will be High Point) with on-call responsibility.
 
 
 
 CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
 
 
 
 The Nurse Practitioner is responsible for delivering comprehensive primary care, palliative care, and chronic disease management to patients in their homes. Conducts physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers. Focuses on health promotion, disease prevention, and patient education to ensure continuous and holistic care for home-bound patients. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
 
 
 Responsible for building and maintaining therapeutic relationships with patients and caregivers, including providing emotional support and education, is a key aspect of this role. Collaborates closely with the interdisciplinary team and effectively communicate, both verbally and in writing, with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations. Participation in on-call coverage for the Integrated Health Services population is required as assigned.

 Graduate of an accredited School of Nursing
 </li>
 <li>
 Current Nurse Practitioner license in North Carolina.
 </li>
 <li>
 Minimum of two years of experience in an acute care, home health, long-term care, or outpatient/office setting required.
 </li>
 <li>
 CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
 </li>
 <li>
 Palliative care experience preferred.
 </li>
 <li>
 Excellent assessment and clinical skills, with strong interpersonal and communication abilities.
 </li>
 <li>
 Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
