Job Summary

The Home Health Nurse is responsible for administering methadone as prescribed by the Medical Director or Clinic Physician and ensuring medication accountability. This role involves supporting the treatment and recovery process by coordinating with healthcare providers, making appropriate referrals, and performing health assessments. The nurse will play a key role in promoting wellness and recovery through community outreach and reducing illicit drug use.

Key Responsibilities:

<li><ul><li>Provide case management services, including advocacy for patients, family education, and referrals to community resources.</li>

Welllness Nurse (RN)

SIGN ON BONUS: $1000 paid out after 90 days of employment The Counselor is responsible for assisting patients in identifying, maximizing, and relating their strengths to appropriate social, educational (academic and vocational), and occupational goals. The Counselor is also responsible for helping the patients prepare and adjust to a drug free existence, and to offer assistance (emotional and physical) in problematic issues. The Counselor is responsible to the Counselor Supervisor or Program Director for provision of services to assigned patients and adherence to schedules, established program policies and procedures, and applicable federal, state, local and accrediting agency re5ulatory requirements. The Counselor will perform duties within the scope of their license or certification which are an addendum to this job description. Credentials: Must have a bachelor's degree in human services or related field. LCAS or LCAS-A required. Summary of Essential Job Functions The key responsibilities of the Counselor include but not limited to: Duties and Responsibilities Conduct alcohol and drug abuse counseling sessions on a regular basis per state regulation, or as part of the individual treatment plan Prompt and regular attendance at an assigned work location. Locations will vary based on Mobile Unit Route. This may require being transported to different location on the Mobile Unit from a centralized starting point. Job Summary

The Program Director oversees the daily operations of a treatment program, including the direct supervision of the treatment center&rsquo;s team members. This role ensures optimal performance in operations, staffing, training, compliance, and customer service. Key responsibilities include scheduling, supervising team members, facilitating training and professional development, overseeing the delivery of treatment services, evaluating patient progress, implementing company policies and procedures, and ensuring adherence to state, federal, and accrediting agency regulations. Additionally, the Program Manager will perform duties within the scope of their license or certification, as specified in an addendum to this job description.

Job Summary
The IOP Counselors emphasizes delivering compassionate, evidence-based care to patients overcoming addiction, using a multidisciplinary approach that fosters hope, respect, and recovery. Responsibilities include individual and group counseling, admissions using ASAM criteria, and maintaining accurate, organized patient records while ensuring compliance with all regulations. Collaboration with the recovery care team and clinical supervisor is essential to empower patients to achieve their best functioning and address underlying health conditions.

<li>Employ an individualized and multidisciplinary approach to care with all medical staff, supporting the &ldquo;shared&rdquo; patient with hope, respect and care</li>
<li>Help patients identify and seek treatment for underlying mental and physical health conditions that perpetuate addictive behavior with a focus on progress and recovery</li>
<li>Work in close partnership with the recovery care team and clinical supervisor while conducting individual and group counseling sessions as required; make referrals to case management services as appropriate</li>
<li>Orient patients to the treatment program&rsquo;s services, operation and procedures; Provide intake/readmission services, including several weeks of supportive follow-up counseling for new patients until a primary counselor has been assigned</li>
<li>Ensure patient records are up-to-date, accurate, complete and well-organized</li>
<li>Comply with all regulations to include State and BHG requirements for quality patient care; respect confidentiality and privacy with the utmost reverence</li>

Qualifications:

The IOP Counselor must have the following qualifications.

Job Summary
The Clinical Care Coordinator service adopts a &quot;whole-person&quot; approach to address the client&rsquo;s needs related to physical health, behavioral health, and social determinants of health. CCC involves the coordination of care between the behavioral health clinician and the clinical personnel of an external provider (e.g., primary care, another behavioral health provider, hospital) Clinical Care Coordination occurs when the practitioner via direct face-to-face contact, video-conferencing, or telephone communicated treatment needs, assessments, and treatment information to external healthcare providers and facilitated appropriate linkages with other healthcare professionals, including transitions into or from a higher level of care or institutional settings. CCC also includes treatment planning and plan of care implementation activities that are separate from the diagnostic assessment service when the clinician and client are meeting face to face or via video conferencing. The Clinical Care Coordinator is responsible for ensuring that the client is at the appropriate LOC. If the clients fail to make progress or have met all of their treatment goals, it is the Clinical Care Coordinators&rsquo; responsibility to ensure timely assessment and transfer to a more appropriate LOC.

Duties and Responsibilities

<li>Facilitate outreach to establish referral providers and participate in outreach events</li>
<li>Facilitate intake admissions and reassessments as scheduled</li>
<li>Board approved and active licensure in the District of Columbia (assigned credentials outlined in Job Description)</li>
<li>Prompt and regular attendance at an assigned work location.</li>
<li>Ability to complete [or oversee the completion of] assigned tasks/projects in a timely manner.</li>
<li>Carry a caseload</li>
<li>Determine the frequency of ongoing Diagnostic Assessment and Plan of Care services</li>
<li>Collect available social, educational, economic, and vocational information to be used as part of the assessment process</li>
<li>Implement or revise patient treatment plans with the patient as required per state regulation and record a review of the patient&rsquo;s progress according to BHG policy</li>
<li>Develop within (7) days of entry or as required by state guidelines, whichever is more stringent, a formal treatment plan documenting patient goals and update utilizing the same guidelines</li>
<li>Maintain patient records that are complete, comprehensive, and well organized</li>
<li>Submit records for periodic review as requested by the supervisory team</li>
<li>Document all patient contacts. If other team members provide services to the patient, the assigned counselor is responsible to see that such contacts are documented in the patient&rsquo;s chart</li>
<li>Make appropriate referrals through case-management services and perform necessary follow-up</li>

<li>Hold a basic understanding of alcohol/drug abuse and addiction. Additionally, keep up- dated on current drug abuse and treatment information, especially current trends in Opioid Maintenance Treatment</li>
<li>Provide intake/readmission services, including several weeks of supportive follow-up counseling for new patients until their assignment to a primary counselor</li>
<li>Provide, monitor, and document follow-up calls/contact to all discharging patients for at least 30 days</li>
<li>Provide crisis intervention/support services to patients assigned to other counselors when their primary counselor is unavailable</li>
<li>Provide inter-agency reports/documentation required for third party reimbursement, criminal justice proceedings, supplemental income assessments, emergency services, and other specialized treatment needs of patients as applicable</li>
<li>Coordinate care with other community providers as needed</li>
<li>Other duties as assigned by supervisory team</li>
