Job Summary:</b></p>
<p>
As a Case Manager at Behavioral Health Group, you will assist patients in achieving their social, educational, and occupational goals. You will provide referrals, coordinate care, and adhere to all regulatory requirements, reporting to the Counselor Supervisor or Program Director.</p>

Key Responsibilities:</b></p>

<li>Orient patients to treatment program operations and procedures.</li>

<li>Make appropriate referrals and perform follow-ups.</li>
<li>Assist in implementing new programs and advise on caseload management.</li>
<li>Provide crisis intervention and inter-agency documentation.</li>
<li>Coordinate care with community providers.</li>
<li>Achieve annual goals and develop knowledge of addiction and related regulations.</li>
<li>Participate in in-service trainings and attend relevant conferences.</li>
<li>Adhere to all federal, state, and local regulations and accrediting agency requirements.</li>
<li>Participate in community and public relations activities.</li>

<p><b>Minimum Requirements:</b></p>
<ul>
<li>Must have a Bachelors degree in the related Human Services field.</li>
<li>Pursuing licensure as a Certified Alcohol and Drug Counselor (CADC) is preferred.</li>
<li>Compliance with federal and state certification and licensure regulations.</li>
<li>Knowledge of addiction, treatment methods, and current trends in opioid maintenance treatment preferred.</li>

<p>
The Nurse Practitioner works under physician supervision, providing medical services including patient histories, physical exams, and treatment plan implementation. Duties include mentoring, professional development, training, and ensuring compliance with regulatory standards.</p>
<p><b>
Responsibilities:</b></p>

<li>Conduct patient assessments, physical exams, and order diagnostic tests.</li>
<li>Develop and implement treatment plans aligned with clinical protocols.</li>

Responsibilities include team member supervision, scheduling, training, service delivery oversight, patient progress evaluation, policy implementation, and regulatory adherence. The Program Director performs duties within their licensed scope, as specified in an addendum.</div>

Reporting to the Program Director, the Clinical Supervisor ensures adherence to schedules, program policies, and regulatory requirements.</p>
