 A day in the life of a Patient Care Coordinator:

 Greets everyone who enters the clinic in a friendly and welcoming manner.

 Schedules new referrals received by fax or by telephone from patients, physician offices.

 Verifies insurance coverage for patients.

 Position Summary:
 
 
 
 The Patient Care Coordinator - I (PCC-I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures. The PCC-I position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.
 
 
 
 Responsibilities:
 
 
 
 
 This position will require traveling between Bear, DE and Middleton, DE to assist at both clinics as needed.

 Convert referrals into evaluations
 </li>

 Schedule patient visits

 Make all welcome calls

 Monitor and influence arrival rate through creation of a great customer experience

 Manage schedule efficiently

 Manage document routing

 Qualifications

 High School Diploma or equivalent
 </li>
 <li>
 Communication skills &ndash; must be able to relate well to Business Office and Field leadership
 </li>

 Ability to multitask, organizational detail, ability to meet deadlines, work with little to no supervision

 As a member of a team, must possess efficient time management and presentation skills

 Physical Requirements:
