<div>Physical Therapist Assistant- Home Care Full Time and PRN 
Requirements and Responsibilities: 
<ul>
<li>
Assists one or more Physical Therapists in providing treatment to help patients recover from injury, disease, or surgery in a home care setting.</li>
<li>Prepares patients and equipment for therapy and implements treatment plans specified by the therapist.</li>
<li>Instructs the patient and guides them in exercises and stretching.</li>
<li>Provides hands-on therapy and treatments.</li>
<li>Records patients' reaction to treatment and provides status reports at the end of each treatment.</li>
<li>Requires graduation from an accredited physical therapist assistant program.</li>
<li>Requires a state physical therapist assistant license in Ohio at least.</li>
<li>Reports to physical therapist.</li></ul>

Position Summary: 

Physical Therapist- Home Care Full Time 
Requirements and Responsibilities:

Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.</li>
<li>Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.</li>
<li>Works cooperatively with physicians, case managers and treating therapists. 
</li>
<li>Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members. 
</li>
<li>Requires state license to practice physical therapy.</li>
<li>Reports to manager of clinical services.</li></ul>
