BSJ Care Center is looking for Full or Part Time LPN's on the PM or NOC Shift. The Staff Nurse LPN will be responsible for assessing individual resident&rsquo;s needs, as well as planning, implementing, and evaluating care for a defined group of residents, under the direct supervision of a Registered Nurse. LPNs coordinates many of the services that affect residents and their families in the protection and promotion of wellness.</b></div></h1><ul><li><div>
Provides professional nursing care to the residents in their assigned area and in other areas as needed.
</div></li><li><div>Administers medications and does treatments as ordered by a Physician.</div></li><li><div>
Reviews Physician Orders and ensures accuracy of Medication Sheets.</div></li><li><div>
Assists in the development of resident care plans.</div></li><li><div>
Goes on routine rounds with all patients in assigned unit notes any changes in condition.</div></li><li><div>
Documents all information regarding a resident&rsquo;s physical, emotional, social, and spiritual wellbeing in the electronic charting system. Charting is done electronically in all Bethany St. Joseph Corporation Facilities.</div></li><li><div>
Prepares residents for admission, transfer, or discharge.</div></li><li><div>
Instructs residents and their families in self-help and self-care.</div></li><li><div>
Supervises C.N.A.&rsquo;s working in assigned unit and directs them in their job duties.</div></li><li><div>
Ensures that resident rights are maintained at all times and reports any suspected violations immediately according to facility policy.</div></li></ul><h2 class="jobSectionHeader"><b>

Requirements:</b></div></div><ul><li><div>
Must be currently licensed to practice in the state of Wisconsin as a Licensed Practical Nurse (LPN / LVN).</div></li><li><div>
Experience working in Assisted Living or Long-Term Care is beneficial, but not required. We will train the right fit for our team!</div></li><li><div>
Must be certified in C.P.R. or have the ability to obtain certification within 6 months.</div></li></ul><p><b>
