From our clinic based team along with clinicians to our at-home care service providers, we deliver high-quality care that empowers patients to manage their health at home.We are currently seeking a full time/ part time Physical Therapist (per diem) for our outpatient private practice located in East Orange, NJ to be responsible for providing professional rehabilitation therapy to patients through evaluation, development, and implementation of individual goal-oriented therapy programs. The incumbent will also be responsible and accountable for the following duties:ESSENTIAL JOB FUNCTIONS:<ul><li>Develop an individualized treatment program based on the evaluation findings focusing on time-oriented, functional and measurable short and long-term goals.</li><li>Complete patient evaluation within the specified period; ensure it contains data that is reflective of a complete, holistic assessment of the client s condition.</li><li>Prepare appropriate and precise documentation, such as progress notes, reassessments and discharge summaries according to policy and procedures; submitted in a timely manner to supports the need for continued skilled.</li><li>Act as liaison between the physician, MCP and other members of the health care team to ensure effective coordination, and appropriate resource referral.</li><li>Ensure service delivery follows the frequency and duration of the physicians plan of care for the patient.</li><li>Provide comprehensive education to patients and/or caregivers based upon their identified needs in a manner that they are able to understand.</li><li>Create a rapport with the patient to foster an optimum environment for goal achievement.</li><li>Document case communication with other members of the health care team as appropriate throughout the patients course of care utilizing secure messaging through an appropriate application.</li><li>Complete scheduling requirements timely and accurately by updating and submitting new and changed schedules through appropriate system applications.</li><li>Implement the treatment plan utilizing correct and appropriate clinical skills and techniques.</li><li>Participate in discharge planning throughout the patient s course of care; address problem areas appropriately.</li><li>Confirm discharge summary indicates the patients status, outcomes achieved and recommendations for further services or interventions.</li><li>Independently meet productivity requirements on a consistent basis.</li><li>Demonstrate flexibility with job responsibilities in all areas.</li></ul>QUALIFICATIONS:<ul><li>A Certificate from an Accredited Physical Therapy Program (APTA).</li><li>An active Physical Therapy license issued by the state of New Jersey.</li><li>Quick-learning ability, and able to adapt to changing patient needs.</li><li>A strong sense of accountability for improving the lives of our patients and their caregivers</li><li>An exceptional focus on teamwork.</li><li>The ability and passion to deliver the highest quality of care, based on a strong sense of patient focus</li></ul>Job Types: Full-time, Part-time, Contract, Per diemPay: $45.00 - $68.00 per hourBenefits:<ul><li>Health insurance</li><li>License reimbursement</li><li>Opportunities for advancement</li></ul>Work Location: In person
