Nurse Practitioner / Physician Assistant - Primary Care

Position Overview:

The Temporary Geriatric Behavioral Health Care Manager is a core member of the collaborative care team, which includes a Bluestone's Primary Care Providers, Nurses, Care Managers and other medical professionals. The Behavioral Health Care Manager is a patient-focused role, primarily working with a complex geriatric population and their families within their home environment (Assisted Living, Memory Care, and/or Independent Living). Additionally, this role is an expert in the field of dementia care, Alzheimer's, memory loss, and/or mental health targeting interventions that improve the patient's overall quality of life.
Schedule: Full time position, day shift hours, no evenings, weekends or holidays. Hours are 8am to 5pm Monday thru Thursday &amp; 8am to 3pm on Fridays. Length of temporary position: mid-June through January. 
 
Location: This position entails a mix of work from home, as well as primarily direct patient care mainly throughout the Roseville, Minneapolis, North St Paul, White Bear Lake areas
 
 
Salary Range: $58,000 - $65,000. Salary commensurate with experience.

Qualifications:

Education/Certification/Experience
<ul>
<li>Formal education or specialized training in behavioral health, including social work, nursing, psychology, gerontology, music therapy or related fields</li>
<li>One or more years of experience in dementia-related care and/or geriatric care</li>
<li>One or more years of direct experience providing mental health support, including utilizing evidence-based approaches for symptom management and improved quality of life</li>
<li>Valid driver's license required</li>
</ul>
Responsibilities:
<ul>
<li>Provide ongoing necessary education and support to patient's care team on Alzheimer's disease and related memory loss/dementia and their impact on cognitive function</li>
<li>Provide ongoing necessary education and support to patient's care team on Mental Illness diagnoses and their impact on cognitive function</li>
<li>Establish care plans that outline interventions to reduce behavioral episodes and improve function and safety</li>
<li>Provide behavioral interventions using evidence-based techniques such as motivational interviewing, problem-solving, modeling, active listening, other techniques as appropriate</li>
<li>Identify and provide de-escalation strategies and crisis resources for caregivers, patients, and families</li>
<li>Provide effective non-pharmacological behavior prevention and reduction solutions</li>
<li>Identify strategies to anticipate and calmly de-escalate distress behaviors</li>
<li>Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications</li>
<li>Complete validated rating scales monthly to monitor and assess response to care plan interventions</li>
<li>Participate in weekly caseload consultations with psychiatric consultants</li>
<li>Facilitate referrals for the clinically indicated services outside of the organization (e.g. mental health specialty care, social services, support groups, etc.)</li>
<li>Act as a Bluestone ambassador for community staff through education and relationship building</li>
</ul>

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around southern Green Bay, WI.

Position Overview:
<ul>
<li>Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities</li>
<li>Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day</li>
<li>Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)</li>
<li>Ability to design and manage your schedule</li>
<li>Develop and implement care plans appropriate to patients and their desires</li>
<li>Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers</li>
<li>Access to diagnostics such as labs and x-rays as needed</li>
<li>Collaborate with community partners within home care, hospice, and other resources as appropriate</li>
</ul>
Qualifications:
<ul>
<li>Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program</li>
<li>Board Certified/Board Prepared through AANP, ANCC, or NCCPA</li>
<li>Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice</li>
<li>Ability to obtain and /or maintain DEA certification for the State of work</li>
<li>Proven ability to communicate effectively with strong verbal skills</li>
<li>Excellent interpersonal and customer service skills</li>
<li>Must have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required</li>
</ul>
Preferred Qualifications:
<ul>
<li>2+ years of clinical experience as a Nurse Practitioner or Physician Assistant</li>
</ul>
