Rapid Re-Housing HMIS Case Worker (1)Implements the activities of the Rapid Re-Housing program in accordance with HUD and the Rapid Re-Housing Program guidelines as directed by the RHP Director.Duties<ul><li>Provides case management for clients utilizing HMIS or a similar system.</li><li>Outreach throughout the service counties.</li><li>Inspects rental property.</li><li>Travels throughout the service areas, must be willing to drive.</li><li>Transports clients when necessary.</li><li>Completes client intake process and develops individual service plans.</li><li>Maintain confidentiality with regards to client information.</li><li>Monitors and evaluates.</li><li>Conducts home visits (minimum two per month) or as needed.</li><li>Completes monthly and annual reports.</li><li>Attends and participates in workshops, trainings, and meetings as approved.</li><li>Completes documents, forms, program files, scans, and administrative duties as assigned.</li><li>Recruit volunteers and solicits in-kind contributions.</li><li>Other duties as assigned.</li></ul>SupervisionThe Rapid Re-Housing HMIS Case Worker is directly responsible to the RHP Project Director for the performance of assigned duties. The RHP Project Director will evaluate the performance of the HMIS Case Worker by monitoring, and by conducting a mid-year progress review and an annual performance evaluation.Qualifications<ul><li>AA Degree (Preferred) or 64 college credit hours or five (5) years&rsquo; experience directly in the field of case work. Degree in Social Work, Social Services, Human Services or a related field.</li><li>Valid Driver&rsquo;s License Required.</li><li>Must be able to successfully pass a Criminal Background Check, a Physical to include TB test, an updated 121 Immunization Record, Drug Screen, and provide an official transcript(s).</li></ul>For consideration, please submit a Cover Letter of Interest, Resume, and Three (3) Professional References via email to: hr@bolivarcaa.org.
