<div>Case Managers Needed Positions available in Manhattan - Bronx &amp; Brooklyn. 

Full time position 
 
Responsibilities:
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li>
<li>Using a person centered approach, provide case management services and assessments rooted in evidence based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.</li>
<li>Utilize electronic health record. Maintain accurate and up-to-date tenant files/charts.</li>
<li>Coordinate and document clinical case management and psychosocial services while also documenting the overall effectiveness of the case management services provided. Case Managers will develop an Individual Service Plan and review the plan periodically to document process toward goals.</li>
<li>Serve as a liaison between tenant and community providers.</li>
<li>Facilitate groups with a focus on life skills, education and support, relapse prevention and mental health counseling.</li>
<li>Complete referrals to medical, mental health, and other outside providers.</li>
<li>Collaborate with Workforce Opportunities Staff to assist tenant in developing employment related goals, where applicable.</li>
<li>Ensure overall compliance with organizational and funder regulations. Adhere to documentation requirements as assigned by leadership and funder regulation/standard.</li>
<li>Coordinate aftercare services and follow-up contacts for post-discharge updates. &Oslash; Conduct outreach and engagement including apartment visits and inspections.</li>
<li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li>

<li>Property Management staff to assist tenant to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors.Mediate landlord-tenant disputes that threaten housing stability.</li>
<li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist tenants with overcoming barriers to housing stability.</li>
<li>Participate in community initiatives to fulfil our mission to end homelessness</li>
<li>Provide crisis intervention as needed.</li>
<li>Provide all services in a manner that respects cultural differences amongst tenants.</li>
<li>Carry out other duties as assigned by leadership.</li>
<li><ul><li>Scatter Site Programs Only: Field work can account for 85% of your time</li></ul></li>
<li>Requirements:
</li><li>A Bachelor's degree in human services or related field.</li>
<li>Must possess strong organizational, writing, communication, and computer skills.</li>
<li>Some relevant experience working with low-income or homeless populations.</li>
<li>Fluent in Spanish is strongly preferred.</li>
<li>Candidates must be self-starters with the skills and energy to work within the community.</li>
<li>Case Manager is regularly required to, type, use hands and fingers, reach with hands and arm. The Case Manager is regularly required to walk, stoop, and kneel. The Case Manager is often required to spend up to six (6) hours sitting at a desk during a normal eight (8) hour shift. Occasionally the Case Manager is required to lift items less than 5 pounds. The Case Manager is regularly required to move throughout the facility while carrying out job duties.</li></ul>

Position Overview</h2>

We are seeking a qualified Phlebotomist / Laboratory Technician to provide clinical laboratory services within the facility&rsquo;s Health Services Department. This role involves direct clinical work including specimen collection, laboratory preparation, documentation, and infection control procedures. The position requires professionalism, attention to detail, and the ability to work in a secure correctional healthcare environment.

Schedule &amp; Work Hours</h2>
<ul><li>Approximately 6 onehour sessions per day</li><li>Up to 30 hours per week</li><li>Monday &ndash; Friday (excluding federal holidays)</li><li>Work hours generally fall between 6:00 AM &ndash; 4:00 PM</li><li>Schedule determined weekly based on facility needs</li></ul><h2 class="jobSectionHeader"> 
Key Responsibilities</h2>
<ul><li>Perform venipuncture and routine blood collection</li><li>Collect specimens including urine, stool, and other diagnostic samples</li><li>Conduct pregnancy screening tests when required</li><li>Perform urine dipstick testing and stool guaiac testing</li><li>Prepare, package, and label laboratory specimens for courier pickup</li><li>Coordinate specimen transfer to outside laboratories</li><li>Maintain laboratory equipment and ensure proper sanitation</li><li>Follow infection control and biohazard disposal procedures</li><li>Maintain accurate laboratory logs and specimen documentation</li><li>Assist with laboratory reports and record management</li><li>Ensure patient identification and proper labeling procedures</li><li>Follow facility safety and clinical protocols at all times</li></ul>

Work Environment</h2>

The services will be performed within a secure correctional healthcare setting. The role may require interaction with individuals in custody during medical testing and specimen collection. Personnel must maintain professionalism, follow all institutional policies, and adhere to security procedures.

Qualifications</h2>
<ul><li>Graduate of an accredited phlebotomy training program or qualified Medical Assistant</li><li>Certified Phlebotomy Technician (CPT) or equivalent certification preferred</li><li>Minimum of one year of clinical, laboratory, or hospital experience</li><li>Experience with venipuncture and specimen handling</li><li>Knowledge of infection control procedures</li><li>Strong attention to detail and documentation accuracy</li><li>Ability to work in a secure healthcare environment</li></ul><h2 class="jobSectionHeader">

The Health Home program is an innovative new model of care management focused on improving the health outcomes for individuals with severe mental illness &amp; other complex chronic illnesses through coordinated networks of medical, psychiatric, behavioral health, social service &amp; housing providers. Essential Duties and Responsibilities: The person filling this position is expected, under general supervision, to: (1) aid individuals and/or families requiring social service assistance; (2) interview and evaluate applicants for services, formulate service plans and goals, and aid clients to implement service plans; (3) locate and make use of appropriate community resources for clients; (4) evaluate actual living conditions of clients though home visits; including the following: 
<ul><li>Maintain professional relationships with clients and client confidentiality.</li><li>Practice Universal Precautions/Standard Protocol &amp; Procedures.</li><li>Comply with any and all Federal, State, City and company security and privacy polices intended to protect the security and privacy of individually identifiable health information.</li><li>Review all documentation establishing clients' eligibility for program and make file copies (e.g. Medicaid status and confirmation of HIV status).</li><li>Assist clients in completing all intake applications and forms.</li><li>Create and maintain client files.</li><li>Carry a caseload of approximately 50 clients.</li><li>Conduct initial intake or assessment of clients and clients' families and/or periodic reassessments.</li><li>Conduct case conferences prior to finalizing all assessments and reassessments.</li><li>Follow-up with clients and with referral organizations regarding client contact and progress with referral organization.</li><li>Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency.</li><li>Recommend and implement strategies to persuade clients to participate more fully in this process.</li><li>Monitor clients' progress toward their service plans goals via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via service plan outcomes and detailed progress notes (i.e. time of service, type of service, etc.).</li><li>Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area, change in Medicaid status).</li><li>Provide all required information for weekly/monthly/quarterly/annual reports (e.g. # of clients in managed care programs, # of clients housed, etc).</li><li>Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, entitlements, etc.</li><li>Escort clients to appointments (educational, medical, social service, etc.).</li><li>Follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.</li><li>May plan, coordinate and facilitate social/peer support events, including group facilitation for clients.</li><li>May prepare marketing materials for the program.</li><li>May input client data and client progress information into automated database.</li><li>May reach out and market the program to the community in order to recruit clients.</li><li>May prescreen clients over the telephone for eligibility and may schedule intake appointments.</li><li>May assist clients in completing applications for benefits and entitlements, and may process applications on clients' behalf.</li><li>Tasks may be modified, expanded and/or assigned over a period of time.

Requirements:
 
<ul><li>Associate's Degree and 4 years applicable experience OR Bachelor's degree (B.A. or B.S.W.) and two years of applicable experience</li><li>Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.</li><li>Bi-lingual English with any one of the following languages: Spanish, Russian, Haitian-Creole, Chinese (Mandarin or Mandarin/Cantonese) preferred.</li><li>Some evening/weekend availability may be required.</li><li>Ability to work in the field depending on needs in the client
