We are currently seeking a compassionate, dedicated Registered Nurse (RN) to join our Admissions Team Monday-Friday 1pm-9pm. What You&rsquo;ll Do:
As an Admissions RN, your primary responsibilities will include:<ul><li>
Assessing patient eligibility for hospice care and explaining the hospice benefit to patients and families.</li><li>
Managing immediate symptoms and providing education to patients and families to ensure comfort and understanding.</li><li>
Collaborating with the attending physician and hospice medical director to develop and implement the initial plan of care.</li><li>
Providing skilled nursing care with a focus on pain control and symptom management.</li><li>
Explaining hospice services and the Medicare benefits clearly and compassionately to patients, family members, and significant others.</li><li>
Ensuring all initial patient and family needs are met with empathy and professionalism.</li><li>
Collaborating with the Interdisciplinary Team (IDT) to address the patient's medical, emotional, and spiritual needs.</li><li>
Building and maintaining relationships with long-term care facilities, hospital staff, and physicians to promote and expand hospice services.</li><li>
Offering guidance on the patient&rsquo;s diagnosis, medications, and treatments.</li></ul> 
Qualifications:<ul><li>
Registered Nurse (RN) with a valid, active license in Pennsylvania.</li><li>
Minimum of 1-2 years of recent hospice experience preferred, but candidates with strong nursing backgrounds will be considered.</li><li>
Valid driver&rsquo;s license required.</li><li>
Excellent communication skills (both verbal and written) with the ability to educate and guide patients and families.</li><li>
Strong problem-solving skills, critical thinking and attention to detail.</li><li>
Ability to perform physical tasks such as lifting, transferring patients, and reaching, bending, kneeling, and standing for extended periods.</li><li>
Basic math skills for medication and dosage calculations.</li><li>
A compassionate and empathetic approach to patient care.</li></ul>
