The Child Development Specialist provides developmentally appropriate home-based and community child development services while collaborating closely with families, caregivers, and interdisciplinary teams to ensure a holistic, trauma-informed approach to care. The Care Manager, Health Homes, is responsible for coordinating comprehensive care for individuals enrolled in the Health Home program. This role focuses on addressing medical, behavioral, and social service needs to improve overall health outcomes.
