POSITION SUMMARY:
 The Nursing Supervisor is a Registered Nurse (RN) who is responsible for the overall supervision of nursing care at High Point Center for Care in both Assisted Living and Memory Care. He/she will ensure the safety and well-being of all residents; provide direct resident care activities to include assessments, planning, implementing and evaluation of nursing care. He/she will provide managerial and/or clinical support to residents, Licensed Practical Nurses (LPN) and Quality of Life Specialists (HHA) to promote good health and achieve high quality of life.
ESSENTIAL JOB FUNCTIONS:
<div><ul><li>Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the resident&rsquo;s needs. Demonstrate good decision-making skills. Provide written evaluation and oral counseling of assigned staff.
</li></ul><ul><li>Assist in maintaining charting and documentation on each resident&rsquo;s status.
</li></ul><ul><li>Provide nursing care according to physician&rsquo;s orders and in conformance with recognized nursing practice, established standards, DOH Regulations, and administrative policies.
</li></ul><ul><li>4.Assists with completing vital signs and treatments as ordered by resident&rsquo;s physician and/or nurse practitioner.
</li></ul><ul><li>Ensure physicians orders are carried out within a timely manner.
</li></ul><ul><li>Ensure Narcotic counts are accurate.
</li></ul><ul><li>Communicate with health care providers to ensure residents have all medication orders and supplies for their care.
</li></ul><ul><li>Communicate with resident family members as needed on all issues/concerns for residents.
</li></ul><ul><li>Participate in the assessment and admission of new residents. Participate in discharge planning as needed.
</li></ul><ul><li>Assist as needed with recruiting, hiring, providing orientation/training, and retaining nursing staff.
</li></ul><ul><li>Responsible for identifying resident medical and behavioral changes and developing and monitoring of residents&rsquo; care plan.
</li></ul><ul><li>Communicate with interdisciplinary team and Health Services Director on pertinent information regarding resident conditions.
</li></ul><ul><li>Assess and monitor complex resident health status and determine the appropriate nursing intervention.
</li></ul><ul><li>Make rounds with physicians and assist in examinations and/or treatment of residents.
</li></ul><ul><li>Perform other duties as assigned.
</li></ul></div>EDUCATION REQUIRED:
<ul><li>Must be a graduate from an accredited school of nursing with a current New York State Registered Nurse (RN) license.
</li></ul><ul><li>CPR Certification is required.
</li></ul><ul><li>Experience with working with Alzheimer's and Dementia residents in an Assisted Living or Memory Care Setting is preferred
</li></ul><ul><li>Two (2) years of supervisory/management experience.
</li></ul>KNOWLEDGE/SKILLS:
<ul><li>The ability to speak, read and comprehend the English language.
</li></ul><ul><li>Ability to learn and work with the specific needs of those residents suffering from Alzheimer&rsquo;s disease and/or other related dementias.
</li></ul><ul><li>Willingness to be available for any/all emergencies regarding the facility.
</li></ul><ul><li>Must possess the ability to make independent decisions when situations warrant such action.
</li></ul><ul><li>Familiarity with Microsoft Office Suite products and the ability to work with an electronic medical record system.
</li></ul><ul><li>Agreement to complete orientation program for Heartfelt Connections Program and other community based training programs.
</li></ul><ul><li>Must display patience, tact, enthusiasm, and a cheerful disposition.
</li></ul><ul><li>Must have knowledge of basic nursing care techniques. Must be willing to learn new techniques and principles and willing to incorporate them into existing resident care practices.
</li></ul><ul><li>Must possess excellent communication and customer service skills
</li></ul><ul><li>Ability to work as a team and display a positive attitude 
</li></ul>SPECIAL REQUIREMENTS:

<ul><li>Lift and carry to 50 lbs. occasionally
</li></ul><ul><li>Pushes and pulls up to 100 lbs. occasionally
</li></ul><ul><li>Climbs, reaches, bends and twists occasionally
</li></ul><ul><li>Talks and listens occasionally
</li></ul><ul><li>Sits 2 hours per day; stands and walks 6 hours per day (combined) &ndash; 8 hr. day
