BronxWorks Homebase assists more than 3600 households with housing retention and stabilization services in the South Bronx. We achieve this by providing services to include case management, public benefits, advocacy, referrals to available resources, mediation, and financial literacy workshops.</div><div></div><div><b><br>
RESPONSIBILITIES:</b></div><div><ul><li>
Review client intakes and determine client eligibility for Homebase services</li><li>
Review cases, to ensure that they meet ESG/EAF eligibility requirements</li><li>
Ensure that ineligible clients are referred to appropriate alternative services</li><li>
Ensure that client records are in compliance with ESG/EAF guidelines</li><li>
Review HRA guidelines with staff to ensure that case managers understand ESG/EAF requirements</li><li>
Review HRA database frequently to ensure that case management staff is complying with HRA quality assurance requirements</li><li>
Conduct weekly supervision with staff and complete supervision forms</li><li>
Complete individual monthly reports for line staff</li><li>
Review Financial Assistance Requests</li><li>
Assess with case managers the risk of clients entering shelter system</li><li>
Review and sign all client service plans</li><li>
Review and sign documentation in client files, where supervisory signature is required</li><li>
Conduct Quality Assurance reviews to ensure cases and case records (paper and electronic) meet ESG/EAF standards</li><li>
With Program Director, create disciplinary plan of action for staff not meeting their job requirements</li></ul></div><div></div><div><b><br>
Education &amp; Training</b></div><div><ul><li>
Participate in both internal and external training.</li><li>
Complete annual certifications (Mental Health, Mandated reporter<b>)</b></li><li>
Maintain knowledge of client services (PA, Medical, Mental health)</li></ul></div><div></div><div><b><br>
QUALIFICATIONS:</b></div><div><ul><li>
Completed master&rsquo;s degree required</li><li>
Three to five years of experience providing case management services preferred</li><li>
Prior supervisory experience working in a social service agency preferred</li><li>
Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment</li></ul></div><p></p><div><br>
