What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:
 <ul>
<li>Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.</li>
<li>Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.</li>
<li>Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.</li>
<li>Continuous visits and interdisciplinary assessment of the patient.</li>
<li>Deliver quality care and completes clear, concise, and accurate clinical documentation.</li>
<li>Provide patient, caregiver and family counseling and education as indicated in the plan of care.</li>
<li>Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.</li>
</ul>
What it takes to be an RN Case Manager with Care Initiatives Hospice:
 <ul>
<li>Active Iowa RN license.</li>
<li>Active CPR certification.</li>
<li>Hospice or palliative care experience.</li>
<li>Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.</li>
<li>Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.</li>
<li>Home access to the internet.</li>
<li>Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.</li>

Business Office Manager (BOM)

Care Initiatives is currently hiring a full-time Scheduler to join our internal staffing agency division, AffiniCare!
 
The Scheduler is responsible for creating a monthly staff schedule and ensuring that each shift is staffed appropriately following local, state and federal guidelines. This position requires exceptional time management, organizational, and communication skills, proficient in Excel, 2+ years of experience is preferred.
 
The Scheduler generates, manages and updates master schedules while making adjustments as needed according to budgetary constraints, census fluctuations, call-ins, and pre-planned leaves of absence. Additional duties include:
 <ul>
<li>Reviewing requests for paid time off (PTO), vacation, maternity leave, Family Medical Leave Act (FMLA), and unpaid leaves of absence</li>
<li>Producing the master nursing schedule</li>
<li>Fulfilling staffing vacancies in advance to ensure adequate coverage for all shifts</li>
<li>Keeping meticulous attendance records and assisting with the progressive disciplinary process if staff members accrue excessive absences and/or tardiness</li>
<li>Communicating with unit managers and the director of nursing / chief nursing officer regarding floor orientation scheduling for newly hired employees in the nursing department</li>
<li>Checking time clock punch records against master schedules and noting the appropriate changes</li>
<li>Signing time correction forms for events such as missing meal periods or forgetting to clock in/out</li>
<li>Utilizing computer software to prepare, change and update the master schedule</li>
</ul>
What it takes to become a Scheduler with Care Initiatives: 
<ul>
<li>High school education or its equivalent. Associate degree, preferred.</li>
<li>Exceptional time management, organizational, and communication skills.</li>
<li>Proficient in Microsoft Office, specifically with Excel.</li>
<li>2+ years of experience is preferred.</li>
