Enjoy the rewarding experience of delivering personalized care while working in vibrant urban communities.Responsibilities<ul><li>Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support</li><li>Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals</li><li>Assist with medication administration following prescribed protocols and maintaining accurate medical documentation</li><li>Support patients with specialized needs such as dementia care, Alzheimer&rsquo;s care, memory care, or hospice support</li><li>Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively</li><li>Maintain detailed records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations</li><li>Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment</li><li>Collaborate with nursing staff and other healthcare providers to coordinate patient care plans and document progress accurately</li></ul>Requirements<ul><li>Valid Home Health Aide (HHA) or Personal Care Assistant (PCA) certification required</li><li>Prior experience in home health, assisted living, nursing homes, or long-term care settings preferred</li><li>Experience working with individuals with disabilities or developmental disabilities highly desirable</li><li>Ability to assist with complex medical tasks such as catheter care, medication administration, and hospice support</li><li>Comfort using medical equipment like Hoyer lifts and performing vital signs assessments</li><li>Strong communication skills to effectively observe patient conditions and collaborate with healthcare teams</li></ul>Join us to provide compassionate home-based care that transforms lives!
