Requirements:<ul><li>
2 years LVN experience, home health preferred</li><li>
LVN license, required</li><li>
IV Therapy/Blood Draw certification required</li><li>
Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)</li><li>
Experience with Kinnser (WellSky) EMR preferred</li><li>
Experience with Allscripts preferred</li></ul>
Primary responsibilities will be:<ul><li>
Serving as a point of contact throughout the admissions process</li><li>
Ensuring coordination of care with other team members and marketing team</li><li>
Communicating effectively with patients, family members, case managers, healthcare providers and co-workers</li><li>
Providing education to patients and family members regarding their home healthcare experience</li><li>
Providing an outstanding admissions experience for our patients</li></ul>

Receive and appropriately document new referrals in a timely manner.</li><li>
Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team.</li><li>
Provide effective communication to patients, staff members, other health care professionals and referral sources.</li><li>
Respond to agency and patient needs in a professional and creative manner.</li><li>
Demonstrate commitment, professional growth and competency.</li><li>
Understands and applies a smooth and efficient admissions process and an outstanding experience for our customers</li><li>
Is able to multitask and has excellent organizational skills</li><li>
Has outstanding communication skills (written, verbal and non-verbal)</li><li>
Autonomous and self-motivated</li><li>
Demonstrates professionalism in all interactions</li></ul></div>
