<li>Recording treatment charges accurately and ensuring that documentation is consistent with billing data.</li>
<li>Writing specific, objective documentation that is outcome oriented and details the patient&rsquo;s needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals.</li>
