The Carolina Outreach team is looking for a compassionate, dedicated, and skilled Psychiatric Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this pivotal role, you&rsquo;ll play a hands-on part in transforming lives by delivering critical, community-based mental health services to individuals with complex needs.
As an ACT Nurse, you&rsquo;ll be at the heart of service delivery&mdash;partnering closely with clients, families, and an interdisciplinary team to ensure that every individual receives the care and support they need. You&rsquo;ll oversee the medical and clinical components of treatment, blending your clinical expertise with a deep commitment to person-centered care.
Each day, you&rsquo;ll bring your clinical skills to life by managing medication monitoring, delivering psychopharmacologic care within your scope, and coordinating services that help clients thrive in their communities. You&rsquo;ll respond to crises, support transitions into and out of care, and collaborate on individualized treatment plans. With your help, clients won&rsquo;t just navigate the system&mdash;they&rsquo;ll make meaningful progress toward stability and recovery.

Compensation: $72,665.72 - $82,472
Position Description:<ul><li>
Monitor and oversee all medical/clinical aspects of services delivered.</li><li>
Provide medication monitoring and documentation.</li><li>
Deliver psychopharmacologic and medical services within your scope of practice.</li><li>
Collaborate with ACT team members to monitor clinical status and response to treatment.</li><li>
Conduct crisis assessments, symptom management, and intervention planning.</li><li>
Provide individual counseling, substance abuse treatment, and psycho-educational support.</li><li>
Assist in creating transition plans when clients move in and out of services.</li></ul>
Minimum Requirements:
<ul><li>Registered Nurse must be licensed to practice in NC by the Board of Nursing.</li><li>
A valid driver&rsquo;s license and current automobile insurance is also required.</li><li>
Each ACT team member must complete DMH-approved ACTT training.</li></ul>
Other Requirements:
<ul><li>Must demonstrate competence in providing ACT services prior to service provision.</li></ul>
