As the <b>Harm Reduction Case Manager</b>, you will play a pivotal role in individuals' lives, making a significant impact through:</p><ul><li>
Conduct outreach activities to engage the target population</li><li>
Identify community partners to engage PWUD who are unaware of their status or untreated</li><li>
Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks</li><li>
Provide enrollment into the agencies Syringe Exchange Program</li><li>
Facilitate brief assessments with the target populations</li><li>
Through review of the assessment, identify community based services to provide client linkage</li><li>
Facilitate linkage to services through escort as necessary</li></ul><p></p><p><b><br>
Harm Reduction Case Manager</b> Minimum Salary: $42,000</p><p><b>
Harm Reduction Case Manager</b> Maximum Salary: $42,000</p><p><b>
Harm Reduction Case Manager</b> Schedule: Mon &ndash; Fri 8:30am &ndash; 4:30pm</p><p></p><p><b><br>
Qualifications:</b></p><ul><li>
High School Diploma or GED</li><li>
1-3 years of relevant experience, Human Services</li><li>
Must have valid Driver's License and access to reliable vehicle</li></ul><div></div><div><div><b><br>
