The Rapid Re-Housing Case Manager is a member of the Rapid Re-Housing service team for the Rapid Re-Housing Program of Catholic Community Services in Thurston County. This position maintains a caseload of single adults that qualify. This includes receiving referrals from Coordinated Entry, assessing housing needs, providing prompt financial assistance as needed for housing costs, and assisting clients to secure housing. The case manager actively uses Housing First and progressive engagement while interviewing and working with client household. In addition, the case manager will enter client data into the Homeless Management Information System. In collaboration with the service team, the Case Manager is responsible for developing and implementing client-defined, goal-oriented Housing Stability Plans, assessing the needs of referred households, utilizing community resources to meet identified individual needs with the goal of stabilizing permanent housing in the shortest time possible. This position works closely with the supervisors and provides support as requested with the overall objective to provide outstanding case management to individuals served by Catholic Community Services.</div><div><br>
</div>Responsibilities:
<h1 class="jobSectionHeader"><b>MAJOR DUTIES AND RESPONSIBILITIES</b></h1><ul><li><b>
Receive referral from Coordinated Entry ( Family Support Center)</b>:</li></ul><ul><li>
Schedule initial intakes within 48 hours of referral.</li><li>
Verify homelessness in writing (3rd party or self-declaration) assure appropriate documentation is obtained to include but no limited to income, SS cards, birth certificates, and ID.</li></ul><div><b><br>
 2. Complete thorough intake with referred clients.</b></div><p>
 Employee accomplishes this responsibility by completing the following tasks:</p><ul><li>
Provide to client short term comprehensive navigation services through Progressive Engagement. This may include walking with client through the system, and modeling behavior for client to include self-advocacy, goal setting, time management, working with agencies, landlords and other needs as called out in the Housing Stability plan;</li><li>
Maintain a weekly housing list of all currently enrolled client households and those presumed eligible;</li><li>
Schedule initial intakes/screenings within 48 hours of referral. Assess client&rsquo;s housing needs create Housing Stability Plan, as well as strengths and supports available to client.</li><li>
Ask referred household what assistance they need to resolve immediate housing crisis.</li><li>
Determine financial assistance to be provided as needed based on household&rsquo;s stated need and program parameters.</li></ul><div><b><br>
 3. Develop and Initiate Housing Stability Plan.</b><br>
 Employee accomplishes this responsibility by completing the following tasks:</div><ul><li>
Identify and prioritize top housing needs.</li><li>
Identify individual strengths/assets to help meet identified needs.</li><li>
Develop action plan for short-term intervention to resolve housing crisis, use of community resources, and family strengths to create a path to permanent housing stability.

Assist client in finding housing within appropriate parameters (size, location, Fair Market Rent);</li><li>
Negotiate with landlord/property manager as needed;</li><li>
Request Housing Quality Standard Inspection to ensure meets minimum HUD standards;</li><li>
Complete request for payment in a timely manner for rent, deposit, other financial assistance as needed;</li><li>
Acquire other documentation (copy of lease);</li><li>
Perform 90-day recertification as needed.</li><li><b>
Assist with locating and securing housing.</b></li></ul><p><b>

Search for landlords that will work with a Rapid Re-Housing program</li><li>
Explain RRH program in efforts to create a collaborative relationship with landlord;</li><li>
Case manager and client are also responsible for helping to locate housing;</li><li>
Respond to Landlord in a timely manner for the following: client emergencies, rent, and deposit and lease issues;</li><li>
Negotiate with landlord/property manager as needed;</li><li>
Schedule HQS inspection with Pierce County and confirm complete prior to payment or move in;</li><li>
Write purchase orders in a timely manner for rent, deposit, other financial assistance as needed;</li><li>
Acquire other documentation (copy of lease, W-9</li><li><b>
Provide data entry.</b></li></ul><p>
 Employee accomplishes this responsibility by completing the following tasks:</p><br>
<ul><li>
Complete all mandatory Homeless Management Information System (HMIS) training;</li><li>
Complete intake, gather all mandatory documents, work with Homeless Services Data Specialist to coordinate entry into HMIS;</li><li>
Enter all ongoing service transactions into HMIS.</li><li><b>
Update and carry out client Housing Stability Plan.
</b><br>
 Employee accomplishes this responsibility by completing the following tasks</li></ul><ul><li>
In conjunction with client household, create and coordinate a Housing Stability Plan based on client directed needs. Plan is to be updated on an on-going basis;</li><li>
In collaboration with community resources, help client as needed to meet identified needs through direct services or referrals to appropriate agencies or programs.</li><li>
With team, re-assess client eligibility prior to 90 days.</li><li><b>
Maintain records and reports.</b></li></ul><div>
 Employee accomplishes this responsibility by completing the following tasks:</div><br>

Keep accurate and timely case management notes;</li></ul><ul><li>
Help create and coordinate a Housing Stability Plan based on client directed needs upon intake. Plan is to be updated on an on-going basis;</li><li>
Complete recertification paperwork as applicable;</li><li>
Complete quarterly rent calculations;</li><li>
Keep organized and confidential records;</li><li>
Participate in RRH case management meetings and provide case summaries and client status information;</li><li>
Participate in problem solving activities with staff members concerning obstacles faced in service delivery and program paperwork compliance.</li><li><b>
Attend meetings as necessary and represent CCS Rapid Re-housing Program.</b></li></ul><div>
 Employee accomplishes this responsibility by completing the following tasks:</div><br>

Participate in CCS/HEN case management meetings and provide case summaries and client status information.</li><li>
Attend appropriate training workshops and opportunities to continually improve knowledge and skills.</li><li>
Share appropriate information with other staff.</li><li><b>
Navigating Community Resources:</b></li></ul><p>
Employee will accomplish this responsibility by completing the following tasks:</p><ul><li>
Physically accompany or transport households for the following: meeting with potential landlords, meeting with community resources such as employment, food banks, mainstream benefits, doctor appointments</li><li>
Maintain current information on community resources;</li><li>
Establish, build and share community resources to include referrals and interactive supportive relationships;</li><li>
Work with community partners to create and ease of communication and warm handoffs.</li><li><b>
Crisis Intervention:</b></li></ul><p>
Employee will accomplish this responsibility by completing the following tasks:</p><ul><li>
Attend training specific to Suicide Prevention, Trauma Informed Care and safety while in the field as well as all other mandatory training;</li><li>
Knowledge of community-based programs specific to the population being served- PATH, Adult Protective Services, Child Protective Services, Mental Health Crisis teams, Detox beds, Emergency Shelters, food programs, health clinics, McKinney Vento etc;</li><li>
Be flexible in the face of changing client needs.</li><li><b>
Team Development:</b></li></ul><p>
Employee accomplishes this responsibility by completing the following tasks:</p><ul><li>
Mentor new and existing staff and intern around RRH policy changes, paperwork and resources to include best practices;</li><li>
Be well versed in CCS system for client referrals as well as working intra-agency;
</li><li>Work collaboratively with the following Family Housing Network programs: Rapid Re-Housing, Permanent Supportive Housing, Coordinated Entry, Shelter and Catholic Housing Services.</li><li><b>
Fiscal Management</b>:</li></ul><p>
Employee accomplishes this responsibility by completing the following tasks:</p><ul><li>
Knowledgeable of allowable RRH costs related to housing, employment, transportation, and general housing stability;</li><li>
Provide timely paperwork for payment towards client housing, transportation and general housing stability needs;</li><li>
Log all funds on internal spreadsheets to track spending;</li><li>
Be good stewards of funding.</li></ul><div><b>

QUALIFICATIONS</b>:</div><div><br>
 </div><div><b>Minimum Qualifications</b></div><ul><li>
Post-secondary education in a social services field or a minimum of 2-year experience working with individuals and/or families with complex needs.</li><li>
Experience providing services to low-income/no-income households to include knowledge of and ability to access community housing, social, medical, mental, chemical dependency services and other needed resources as appropriate based on client(s) needs.</li><li>
Ability and willingness to work independency as well as in a team environment;
</li><li>Highly motivated self-starter and ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment.</li><li>
Excellent inter-personnel skills including excellent oral and written communication skills;</li><li>
Excellent organizational and time management skills&mdash;Ability to maintain accurate, thorough, timely and complete client files.</li><li>
Computer skills including Microsoft Word and database experience.</li><li>
Ability to work with a diverse spectrum of high-need individuals.</li><li>
Reliable personal transportation, valid Washington driver&rsquo;s license, auto insurance, and an acceptable driving record according to CCS&rsquo; driving policy, required to provide current driving abstract.</li><li>
