The Pharmacy Technician will be the main point of contact for interactions with the pharmacy and will be responsible for carrying out the day to day operations of the pharmacy. The Pharmacy Technician will assist the Pharmacy Supervisor and other clinical team members with clinical programs as needed. Essential Job Functions: Data entry of patient/facility information and prescription orders Customer service (i.e., receiving and processing order requests) Daily fill including controlled substances Prescription fulfillment (i.e., various packaging, ATP2, blister packaging, vials) Staging items for delivery or shipment Resolve billing issues Assist Pharmacist with management of appropriate inventory levels Respond to telephone inquiries, referring to appropriate staff as needed Complete special projects as assigned by supervisor May maintain records related to the use of pharmaceuticals and quality assurance activities as directed May maintain relationships with nurses, physicians, and other health care professionals regarding medication problems for individual patients May participate in the development of and coordinate orientation and training programs for other pharmacy technicians Demonstrate cultural competence by being respectful of and responsive to health beliefs, practices, cultural and language needs of the member and his/her support system Comply with all local, State, and Federal laws with regards to protected health information (PHI), including HIPAA Ensure pharmacy policies and procedures are followed Perform all other duties relevant to the position as assigned by supervisor. Knowledge, Skills, and Abilities Ability to work collaboratively and effectively with interdisciplinary team members, pharmacy patients, families, and providers Ability to communicate effectively, both orally and in writing Ability to organize complex information and make independent decisions Maintain all required certifications/training by State regulations Possess strong computer skills and efficiency using Microsoft Word, Excel, and other applications Ability to efficiently and competently navigate electronic health records (EHR) and electronic medication administration records (eMAR) Education and Experience: High school diploma or equivalent combination of education and experience 1-2 years&rsquo; experience working in Long Term Care, retail, or hospital outpatient pharmacy preferred Certification as a Pharmacy Technician (CPhT) is preferred Ability to work independently and in a team environment Computer, email, fax and scan knowledge All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Executive Vice President, in collaboration with Human Resources. Assist care managers with the interpretation of assessments and screening tools, recommend additional screening or assessment when appropriate, and facilitate access to preventive health screenings. Work in partnership with care managers, assess safety concerns in an individual&rsquo;s environment to promote accessibility and mobility, as well as to provide instruction in patient safety Conduct assessments and assist in developing Life Plans Conduct clinical reviews for individual specific needs or overall program trends and outcomes Conduct home visits for assessment and planning for individuals and families with complex health needs Case conference with clinical providers of treatment to ensure continuity of care Provide health literacy programs and training for staff Provide crisis intervention, referrals and support to individuals and families Provide on call availability with shared coverage with other clinical team members Represent Prime Care Coordination on appropriate committees, boards, and task forces. Establish and maintain positive relationships at all levels inside and outside the organization. Perform all other duties relevant to the position as requested.
